The Pharmacist (Facility Program Manager) PACT is responsible for monitoring, developing and evaluating the activities of the Clinical Pharmacy Specialist staff at El Paso VA Health Care PACT outpatient clinic and CBOCS. General Description Responsible for programs designed to improve prescribing practices and medication therapy outcomes through the application of advanced pharmacoeconomic principles and implementation of clinical practice guidelines in coordination with the Pharmacy and Therapeutics Committee as well as other EPVAHCS and oversight agencies. Responsible for supervision of clinical and supportive staff assigned to area. Responsible for providing education and teaching of health care professionals and health care professionals in training. Responsible for the design, conduct, collaboration, and dissemination of for DUEs and other Serves as active member or guest on facility, VISN and national committees as requested. Clinical Practice Supervises advanced clinical services in a specialty area requiring in-depth knowledge and advanced monitoring skills and assessing patient outcome. Assists the Assoc Chief of pharmacy in ensuring seamless operations in clinic. Interfaces with physicians and others in the care of selected veteran patients to achieve optimization of rational and cost-effective drug therapy. Serves as a resource for advanced information and expertise to other pharmacists and health professionals in a specialty area. Program Management Responsible for coordinating clinical pharmacy staff participation in education programs and activities in conjunction with the Pharmacy Service education and quality manager and section supervisor. Responsible for coordinating clinical pharmacy staff participation in quality assurance programs and activities in conjunction with the Pharmacy Service quality manager and section supervisor. Responsible for coordinating clinical pharmacy staff participation in pharmacy programs to assure compliance with Joint Commission and other regulatory agency requirements in conjunction with the Pharmacy Service quality manager and section supervisor. Providing effective leadership to all assigned staff by promoting Lean and other organizational priorities that improve veteran services, quality, safety, and health outcomes. Reviews clinical utilization workload, pharmacist productivity and reports to the Associate Chief of pharmacy monthly Reviews "Today's Work Today" to ensure notes are signed, and consults are acknowledged. Pharmacoeconomics Applies advanced knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics in making judgments and recommendations of optimizing drug therapy. Serves when assigned as an active member of the Pharmacy and Therapeutics and other Committees through the review of policies and procedures concerning medication use, the implementation of clinical guidelines, the analysis of historical medication use, and forecasting future medication use trends. Responsible for developing and implementing criteria for use of high cost or high-risk medications as well as evaluating the assessment strategies for these use criteria. Responsible for educating providers and pharmacy staff regarding medication use initiatives as well as medication safety issues. Administrative Supervises professional employees assigned to section, including clinical pharmacy specialists, WOC faculty and residents. Assists with evaluations and delivering performance standards. Creates work schedules for clinical pharmacists and may reassign workload or cover for callouts as appropriate. Approves/denies leave requests and consults with the Associate Chief Clinical pharmacy as appropriate. Actively participates in and provides leadership to councils, boards, committees and workgroups at the facility, VISN and national level as assigned. Actively participates in local, state and national organizations related to the advancement of the pharmaceutical profession as assigned. Reports directly to and requires minimal supervision by the assigned Assistant Chief, Pharmacy Service. Work Schedule: Monday - Friday 8:00am - 4:45pm Financial Disclosure Report: Not required
