How would you like to become a part of a team providing compassionate care to Veterans? The Clinical Pharmacy Specialist will work in the Centralized Anticoagulation Service Hub. The clinical pharmacy specialist serves as an independent practitioner in the Centralized Anticoagulation Service Hub, requiring the highest level of pharmacist clinical privileges with minimal supervision. Principal responsibilities include pharmacotherapy management, provision of drug information services, and patient education. Major Duties: Utilizes a scope of practice for Clinical Pharmacy Specialists (CPS) to manage assigned patients. Direct patient care may include, but not be limited to, telephone visits, telehealth visits, chart reviews and use of secure messaging. Serves as an authoritative information source on drugs and their utilization in therapy, with special emphasis on medications used in treatment of the assigned patients. Evaluates the drug literature by analysis of experimental design and methodology, in order to compare and contrast therapeutic regimens and roles for new drugs, with a special emphasis on medications used in anticoagulation. Designs and document in the medical record, a therapeutic regimen or plan, based on patient-specific information, which includes establishing a therapeutic endpoint, selecting an appropriate drug and related therapy, and establishing an appropriate dosage regimen. Participates in adverse drug event monitoring, medication error reporting, medication utilization evaluation activities, drug monographs/reviews, and other activities as assigned. Provides education and training programs to other health care team members, as appropriate. Educates patients on the proper use and care of their medications, and related topics. Interfaces with other providers in the design of the veteran's treatment regimen to achieve optimization of rational and cost-effective drug therapy.
