UPMC in Harrisburg, PA is recruiting for a Solid Organ Transplant Clinical Pharmacist Specialist to join its renal transplant program.
The Clinical Pharmacist Specialist will serve as a part of a multidisciplinary team to advocate for the safe, effective, and fiscally responsible use of medications.
Individuals in this role collaborate with transplant surgeons, transplant nephrologists, advanced practice providers, nurses, case managers and pharmacy leadership to deliver care to both inpatient and outpatient renal transplant recipients and donors. Job responsibilities include but are not limited to direct patient care services, patient and family teaching, provision of medication information, participation in quality assurance and performance improvement activities, evaluation of medication usage and patient-related outcomes, education of all individuals involved in the medication use process, and precepting of pharmacy students and PGY-1 residents.
This position will have a dual reporting role to both the System Director of Clinical Pharmacy Services and Director of Transplantation Services.
Doctor of Pharmacy degree
Current licensure or eligibility for licensure as a pharmacist in Pennsylvania
Completion of an ASHP-accredited PGY-2 in solid organ transplant
BPS Board Certification in Pharmacotherapy
