Responsibilities: The clinical pharmacist is responsible for a high level of direct patient care and assures optimal drug therapy primarily, but not exclusively, for patients with cardiovascular disorders. The pharmacist is responsible for clinical pharmacy services including but not limited to: participating in medical rounds, collaborate with other healthcare professionals, conducting medication reconciliation, reviewing medication orders for compliance with hospital policies, educating staff, and optimizing medication therapy to achieve the best outcomes possible for patients. Additionally, the pharmacist will oversee the medication distribution system for their area of responsibility.
