The Ambulatory Care Clinical Pharmacist Specialist hired for this position will provide comprehensive medication management (CMM) to patients with chronic conditions within the Ascension Medical Group (AMG) Clinics with a primary base of operations being the AMG Gardendale Clinic.   While these services will be available to any patient served by these clinics, the specialist will have a specific focus on improving medication-related population health metrics for Medicare and SmartHealth (Ascension Medical Plan) patients.  This care may be provided to patients under a variety of models including independent consultations with the pharmacist (both face-to-face and telehealth), shared medical appointments, or group visits.   The position will also offer the interested candidate the opportu
