Description JOB SUMMARY The clinical pharmacist will be an integral member of an interdisciplinary healthcare team and provide comprehensive pharmacy services to internal medicine patients at Vanderbilt University Adult Hospital. Activities include but are not limited to actively engaging with providers (rounds, huddles, phone calls), developing specific pharmacotherapy plans, and recommending medication regimens and monitoring plans that are individualized to meet the needs of the patient. In addition, the clinical pharmacist will support the pharmacy department through order verification, answering of drug information questions and formulary adherence.
