Plans, directs, and implements clinical pharmacy services to provide high quality patient care by optimizing drug therapy, minimizing adverse effects and reducing costs to the hospital. Provides consultative services to the medical team; provides drug information to the medical team and patients/care givers; initiates and participates in performance improvement campaigns; provides formal in-services/continuing education to the medical team; participates in precept pharmacy practice residents/students; proactive in implementing TJC standards, hospital policies and procedures and regulatory standards of practice. Responsible for overseeing the provision of comprehensive, patient centered clinical pharmacy services. Initiates, participates and oversees the collaborative management of patients with other members of the health care team, including medication reconciliations, core measure compliance, anticoagulation management, target drug programs, medication usage guidelines, patient teachings, discharge teachings and performance improvement initiatives. Participates in the continuous readiness efforts to maintain The Joint Commission patient safety goals and medication management standards. Participates in developing and presenting educational topics, and staff development programs when requested.
Ensures safe, appropriate and cost effective drug therapies for patients according to established policies and procedures. Assists in the development, implementation and maintenance of clinical pharmacy services. Facilitates the development of treatment guidelines, protocols and formulary changes as approved and authorized by appropriate committees. Provides clinical consultation and clarification to practitioners.
Provides education and information on pharmaceutical care and drug therapy. Provides formal in-services and/or continuing education programs to pharmacy staff, nursing, medical and other health care professionals to enhance the professional growth and support BHS commitment to clinical excellence. Supervises and instructs pharmacy students and residents.
Prepares agenda for the assigned medical staff committees, provides content and presents drug reviews, DUE criteria, drug class reviews, analyzes microbiology data, publishes the antibiogram annually and reports antibiotic sensitivity data to the Pharmacy & Therapeutics Committee. Coordinates and prepares agenda for the Antibiotic Review Committee. Reports to Pharmacy & Therapeutics Committee recommendations and actions of Antibiotic Review Committee.
Evaluates the medical literature for information supporting the development and implementation of strategies, protocols and programs designed to improve, upgrade and advance the quality and effectiveness of pharmacy services. Participates in the preparation and publication of the pharmacy newsletter to inform the staff of clinical updates or procedural changes.
Doctorate Pharmacy - DP. Registered Pharmacist. 5 years of experience.
Preferred Progressive pharmaceutical care experience (or) Pharmacy Practice Residency. Each year of Pharmacy Practice Residency counts for 1 year of experience. Minimum 1 year supervisory experience.
The newly established Clinical Pharmacy Enterprise at Baptist Health South Florida is led by a Chief Pharmacy Officer with the goal of creating progressive clinical pharmacy practices throughout the system. Clinical pharmacy services at Baptist Health will experience sustained growth and expansion. The acute care services will be developed and expanded to create new roles for pharmacist and technicians in many specialty areas. The continuous growth of the Miami Cancer Institute provides opportunities for innovative practice in all oncology subspecialties. The planned development of the newly establish Ambulatory Pharmacy Services will include expansion of clinical pharmacists into ambulatory clinics, retail, specialty and infusion services. A strong commitment to education, interdisciplinary participation and support for the latest in technology will assure that pharmacy staff is in the forefront of the medication management process.
The Clinical Pharmacy Enterprise at Baptist Health is a forward thinking organization with a leadership team that fosters an environment where unique professional growth opportunities for Pharmacists are a priority. Our Clinical Pharmacy Career La...dder has 4 levels of pharmacist roles with growth potential into Clinical Coordinators and Management positions. Our Corporate Residency Director oversees our PGY-1 and PGY-2 pharmacy residency programs with 15 current residency positions and will expand into other specialty areas. We are known for having a high post-residency retention rate because of the support provided to the residents. Future plans are in place to offer non-traditional ASHP-accredited pharmacy residencies by 2021.
