The Pharmacy Service is in need of support in the 24-hour Inpatient Pharmacy Service at the John D. Dingell VA Medical Center. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education or Experience. A high school diploma or equivalent; or at least 3 months experience. Certification. Certification by the National Pharmacy Technician Certification Board (PTCB) is required at grades GS-6 and above English Language. Pharmacy technicians appointed to direct patient care positions must be proficient in spoken and written English accordance with VA Handbook 5005, chapter 2, section D, paragraph 5a, this part. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: One year experience at the next lower grade level (GS-05) and must fully meet the KSAs at the next lower level. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*): (b) Demonstrated Knowledge, Skills, and Abilities *1. Ability to solve problems and make recommendations, e.g., troubleshoot operational problems and refer for appropriate action. 2.Knowledge of the operation and care of automated equipment. 3. Knowledge of, and ability to follow, written instructions for compounding with aseptic technique for hazardous materials such as oncology agents. 4. Knowledge, and ability to perform, compounding with aseptic technique for routine sterile products. 5. Knowledge of policies and procedures for inventory management in specialized areas such as emergency carts. 6. Knowledge of the computerized prescription process demonstrated by the ability to input prescriptions and medication orders. Preferred Experience: Minimum of two years of Inpatient work with preparation of sterile IV compounds. Work with automated dispensing cabinets; Omnicell/Pyxis or similar. Effective communication of procedures and policies or training thereof. Physical Requirements: The work requires regular and recurring physical exertion. The duties include but are not limited to preparing medication orders - controlled substances and non-control substances, inventory management, sterile product compounding including chemotherapy and hazardous drugs, unit dose prepacking, non-sterile product compounding, ward inspections, automated dispensing cabinet stocking and inventory maintenance thereof. The incumbent must demonstrate comprehensive knowledge of pharmaceuticals and pharmacy practices. The incumbent is trained and familiar with departmental procedures and policy guidelines to provide a variety of pharmacy services to Inpatient units and ambulatory care (Outpatient) areas. Provides pharmacy support to CBOC's, and off-site locations that provide Domiciliary and Homeless Outpatient clinic care. The incumbent must maintain a state issued driver license and be able to operate a government vehicle with completion of all required facility training to drive it. The incumbent will be expected to restock controlled substances and non-controlled medications and supplies, provide maintenance of automated dispensing machines, assisting with the monthly unannounced inspections required by the Controlled Substance Inspection Program, entering drug entries with synonym and barcode information for automated equipment, filling new and missing dose requests, prepacking medications to fulfil unit dose orders, filling/monitoring ward stock replenishment, non-sterile compounding activities, handling hazardous medications, receiving inventory, reviewing Omnicell reports, determining par levels, requesting out of stock products, and any other function related to pharmaceutical practices. The applicant will correctly fill CADD pump devices/syringes and accurately fill orders for checking by a licensed pharmacist, manage recall, expired mediation and or prepare such medications for destruction. The applicant work on an independent level and perform complex duties including preparation of oncology/chemotherapy and handle/inventory-controlled substances for final check by pharmacist. The incumbent will function to provide tour coverage during absences for the Controlled substance or Oncology Pharmacy technicians but at no more than 25% of time per VHA 5001. Reports medication safety issues and quality of care issues through the appropriate channels and acts to minimize and or prevent defects in the medication management system. The incumbent will be competent with USP 797 practices for IV admixture services including chemotherapy, pre-operative, and cardiac cath needs. Using aseptic technique, prepares sterile products for compounding (TPN, IVPB, Large Volume Parenteral, ophthalmic) to be checked by a pharmacist. Demonstrates competence in the preparation of low and medium risk sterile products as outlined in USP 797 requirements. Demonstrates competence to prepare chemotherapy and other hazardous drugs. Prepares chemotherapy products following safe medication practices and guidelines for protection from hazardous drug exposure. Day shift technicians may occasionally be required to prepare IV preps for shift and most of entire 24-hour period if staffing and workload requires. Fill controlled substance orders and or patient specific prescriptions. Delivers, receives, and documents transactions according to the policy for this assignment. Reports discrepancies to pharmacist/ supervisor and or the Controlled Substance Coordinator (CSC) in a timely fashion. Performs inventory of the vault in compliance with local and VA requirements. Works cooperatively with the Clinical Pharmacists to problem solve patient care related issues that may arise with preparation and dispensing of chemotherapy and other drugs. Completes training programs on hazardous waste and spill management and Controlled Substances as required in hospital policy and in a timely fashion. Using data from VISTA and Omnicell system, medications are filled and dispensed properly utilizing bar code scanning to indicate the unit of issue. Able to perform basic troubleshooting of automated equipment, perform daily preventative maintenance to ensure safe operation, and safely maintain sufficient drug and supply levels. Follows documentation procedures for Omnicell including entry of expiration dating and use of bar code safety stock to prevent misfills. Utilize knowledge of local procedures and VA Directives to perform day to day functions. The applicant will be expected to perform other duties as assigned.
