The role you’ll contribute:
Primary responsibility is to facilitate the development, implementation, and continuous quality improvement of coordinated pharmacy services across the system. Provides guidance for the integration of clinical pharmacy services, drug utilization programs, and pharmacy operations throughout the institution. Ensures medication use is aligned with evidence-based best practices and regulatory standards. Directs the activities of pharmacists who work within assigned area. Work to meet highly specialized pharmacy business needs in programs such as Clinical Services, Medication Safety, Antimicrobial Stewardship, Education and Staff Development, Supply Chain, Policy, Regulatory, Informatics, Operations Services. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Coordinates, audits, and maintains existing and new pharmacy programs, quality improvement changes, and medication stewardship programs to advance the organization’s medication-use systems and promote optimal patient outcomes.
Coordinates, audits, and maintains existing quality assurance programs, including but not limited to drug utilization reviews; clinical intervention documentation audits; and clinical outcome assessments, to maintain optimal clinical and operational pharmacy services and identify opportunities for growth.
Coordinates and maintains existing education and training programs designed to fully develop the skills of the pharmacy department staff and ensure the competency of the pharmacists involved in any pharmacy services.
Develops and coordinates educational content for medical staff members, nursing partners and ancillary teams
Facilitates the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees.
Provides departmental and institutional representation for medical staff committees/meetings, hospital groups/task forces, and approved external programs/organizations.
Participates and coordinates pharmacy, facility, and medical staff committees as assigned
Acts as a preceptor for pharmacy residents based on the guidelines of the ASHP residency accreditation standards and for pharmacy students on rotation consistent with academic requirements.
Performs other duties as assigned
Policy and Regularity Compliance
Maintain expertise in the standards of The Joint Commission (TJC) and/or Det Norske Veritas (DNV), International Organization for Standardization (ISO) quality management systems, Center for Medicare and Medicaid Services (CMS), Agency for Health Care Administration (AHCA), Board of Pharmacy, and other bodies relevant to hospital practice.
Audits and coordinates pharmacy quality assurance program including pharmacy department compliance with healthcare regulatory and accreditation agency requirements, and applicable state and federal laws across the division.
Proactively use data analysis techniques to track, monitor, and communicate the operational quality outcomes of assigned areas and to benchmark performance against internal targets and against local and national best practices.
Identify, resolve, and report short- and long-term obstacles to patient care and medication safety, regulatory compliance, or stewardship of resources.
Design, implement, and coordinate roll out of new pharmacy programs, quality improvement changes, and continuous quality assurance initiatives to advance the organization’s medication-use systems and promote optimal patient outcomes.
Facilitate the update and maintenance of medication-related and pharmacy-related policies and procedures and prospectively assess new processes and policies for appropriateness and compliance to defined standards.
Prepare agenda, minutes, drug monographs, policy & procedures and formulary recommendations for the P&T Committee
Provide consultation to physicians on drug usage and therapeutic appropriateness decisions.
Develop and monitor medication use and reports results to P&T Committee and Medical staff committees
Design and implement quality and performance improvement initiatives around formulary agents
Review utilization of formulary and non-formulary medication, including but not limited to adherence to criteria for use
Facilitate the development and implementation of treatment guidelines, protocols, formulary and/or criteria for use changes, and critical pathways as approved by appropriate committees.
Facilitate the development or revision of clinical pharmacy services
Antimicrobial Stewardship Services
Coordinate antimicrobial stewardship services to advance the organization’s antimicrobial-use systems and promote optimal patient outcomes.
Partner with providers to support safe, efficient, effective, equitable, timely, and patient-centered use of anti-infectives
Design and implement quality and performance improvement initiatives around anti-infective use
Coordinate education and training programs designed to fully develop the clinical skills of the pharmacy department staff and ensure the competency of the pharmacists involved in clinical pharmacy services relating to antimicrobial stewardship.
Facilitates the development and implementation of infectious diseases related treatment guidelines, protocols, formulary and/or criteria for use changes, and critical pathways as approved by appropriate committees.
Facilitate dialogue with campus leaders to ensure local antimicrobial stewardship programs comply with the Centers for Medicare and Medicaid Services Conditions of Participation, the Centers for Disease Control and Prevention Core Elements of Hospital Antimicrobial Stewardship Programs, and accrediting bodies.
Partner with microbiology department to routinely assess and update antimicrobial testing and reporting panels, including optimization of rapid diagnostics.
Develops the annual antibiogram compilation in collaboration with the microbiology department
Performs ongoing review of anti-infective financial performance, including identification and implementation of budget initiatives during the annual budget planning process.
Collaborate with infection prevention on drug-related initiatives to reduce healthcare acquired infections (e.g., C. difficile, MRSA).
Coordinate preparation of various antimicrobial stewardship metrics, including, but not limited to, antimicrobial days of therapy, standardized antimicrobial administration ratio, antimicrobial cost per adjust patient days, and changes in antimicrobial resistance patterns
Design and maintain technology functions to optimize pharmacy operations, clinical services, automation devices and applications, medication safety, supply chain management, and pharmacy finance processes
Provide leadership in evaluating and prioritizing the development of EHR and automation applications and hardware, and facilitates project management of pharmacy technology solutions to allow for smooth implementation, upgrades, maintenance optimization, and changes in workflow
Maintain expertise in data analytics, project management, pharmacy automation hardware and applications, and electronic health record (EHR) integration and functionality
Maintain proficiency in and active working knowledge of pharmacy operational workflows and pharmacy programs for medication management, procurement, and distribution
Facilitate project management of pharmacy technology solutions to allow for smooth implementation, coordinated upgrades, proactive maintenance optimization, and timely changes in workflow
Administer and support the day-to-day performance of the pharmacy information and automation systems integrated throughout the Pharmacy department operations and clinical programs
Proactively use data analytics to track, monitor, and communicate the operational quality outcomes of pharmacy automation and technology, including benchmarking performance against internal targets and known best practices
Educational and Staff Development Services:
Create, develop, and maintain the structure, format and timeline of pharmacy education and training philosophy, goals, and requirements
Identify, assess, and facilitate the provision of resources and tools for successful execution of education and training programs
Design and maintain the department orientation for all new pharmacist personnel, including residents to ensure the orientation process contains all the required elements the job including applicable computer systems, policy and procedure, customer service and communication etiquette.
Design, maintain, and facilitate education and training programs aimed at developing the clinical skills of the pharmacy department staff and ensuring the competency of the pharmacists involved in pharmacy services.
Serve as a consultant to supervisors and administrators to provide other educational resources and training to meet the specific needs of the Pharmacy department(s).
Coordinate the creation of education programs, including use of eLearning technologies, to enhance the knowledge level of the pharmacy staff. Assists with reviewing and updating established education programs to ensure relevance and accuracy.
Monitor and ensures completion of competencies to meet departmental, institutional, and regulatory compliance needs.
Participate as active member of AdventHealth Education Council and AdventHealth Learning Network (ALN). Serves as key liaison between the Pharmacy department and other AdventHealth education staff.
Medication Safety Services:
Identify opportunities for proactive change in the medication use-system to improve patient safety utilizing medication event-reporting data, published best practice standards, and published events or recommendations.
Lead the review and evaluation of significant medication variances and recommends process/system changes. In collaboration with the medication safety team, risk management team(s) and the pertinent healthcare providers, formulate a plan to address the suggestions for change resulting from root-cause analyses.
Utilize operational knowledge of key medication safety technologies including medication dispensing systems, IV pumps, barcode medication administration, and computerized provider order entry to evaluate medication variances and improve processes.
Assess and reports the status of efforts to improve medication-use safety to campus leaders, decision-makers, and staff. Serves as a member of the Medication Safety Committee and other relevant Hospital Committees focused on medical/medication error reduction.
Develop strategies to minimize the potential for medication errors associated with medication labeling, packaging, nomenclature, storage, standardization, preparation, distribution, dispensing, and administration.
Maintain expertise in the standards of The Joint Commission (TJC) and/or Det Norske Veritas (DNV), Board of Pharmacy, and other regulatory bodies relevant to Pharmacy Operations.
Coordinate implementation of operational process and product changes across the department or system which may include elements such as staff training, inventory management, quality assurance and regulatory compliance.
Provides oversight and continual monitoring of operational processes to include metrics and KPIs.
Recommends changes to operational processes that are standardized
Provide operational leadership by facilitating the development, implementation, and maintenance of hospital and departmental systems that promote desirable patient outcomes
Support education for students, interns, residents, nurses and patients.
Maintain knowledge of and take action upon dashboards reflecting pharmacy productivity, budget and outcomes initiatives.
Maintain proficiency in USP 795, 797 and 800. Understand and implement remediation for personnel and environmental monitoring as necessary.
Communicate with nursing and other external partners regarding operational changes that impact these stakeholders.
Supply Chain Services
Support patient care by coordinating the procurement, storage, preparation, and distribution of drugs to all patients.
Identify changes in drug usage (volume and patterns) based on published literature and prescribing practices at each hospital.
Advise and inform pharmacy leadership regarding drug shortages and mitigation strategies that support patient care.
Maintain knowledge of and act upon information or dashboards reflecting pharmacy productivity, budget and outcomes initiatives
Participate and advise during pharmacy contracting, budgeting and monthly close processes.
Monitor contracts and pricing to ensure purchasing compliance as well as accurate load of data into wholesaler software.
Optimize storage (refrigerated, setting inventory levels, maintaining space, eliminating overstock, avoiding waste, etc.) and distribution
Maintain proficiency with reports and data interpretation (Pyxis, PLX, ABC, KP, excel).
Understand and adheres to 340B regulations as required.
Understand, code, and approve purchase requests as necessary
