Overview Overlook Medical Center is currently seeking a clinical pharmacist within the Pharmacy Department.This position will be a full-time inpatient transitions of care (TOC)/internal medicine (IM) clinical pharmacist for Overlook Medical Center who will collaborate with and serve as a resource for nurses, physicians, pharmacists, and medical residents. The TOC/IM clinical pharmacist will participate in daily multidisciplinary rounds on a dedicated unit, assist in discharge medication reconciliation, and provide patient education.
