Banner Pharmacy Services provides innovative pharmacy services and coordinates patient care across the continuum, leading to improved medication outcomes. This position is a full-time role, working 40 hours per week, to be split supporting the Banner University Medicine Whole Health Clinic at Wilmot in the mornings and serving as a Health Plan Pharmacist for the BUHP pharmacy team in the afternoon.
As an Ambulatory Care Clinical Pharmacist, embedded within a behavioral health specialty clinic, you will have the opportunity to provide disease state management services while working under collaborative drug therapy management (CDTM) agreements. Ambulatory Care Clinical Pharmacists actively manage medication therapy, complete medication reviews, collaborate with providers to optimize medication therapy, improve medication adherence, promote patient activation and wellness, prevent disease, and improve outcomes. As a Health Plan pharmacist working on the BUHP pharmacy team, you will have the opportunity to work on interdisciplinary patient-care rounds, perform drug utilization reviews, and develop and implement clinical programs to positively impact the care of the covered population. A degree in Pharmacy from an accredited College of Pharmacy. Completion of an accredited PGY1 pharmacy residency program or three years of previous clinical pharmacy experience in the ambulatory care setting.
Requires current and unrestricted pharmacy licensure in the applicable state of practice within 90 days of hire date. A current and unrestricted Pharmacist license (not an intern) is required at the time of hire for BSS applicants.
Maintains good organizational, problem-solving, and computer skills. Possesses good written and verbal communication skills for interfacing with all levels of staff, physicians, patients, and other contacts.
PREFERRED QUALIFICATIONS Additional related education and/or experience, including Board Certification in a related field, preferred or PGY2 pharmacy residency program in ambulatory care.
