The Clinical Pharmacist of The Family Health Centers (FHC) is responsible for the effective implementation, integration, and coordinating clinical pharmacy services and ensuring that optimal clinical pharmacy services The Clinical Pharmacist is responsible for the effective implementation, integration, and coordinating clinical pharmacy services and ensuring that optimal clinical pharmacy services are provided daily.
The Clinical Pharmacist facilitates the development, implementation, and maintenance of systems to promote desirable patient outcomes through the use of appropriate, safe, and effective medication therapies using evidence-based standards of care. The Clinical Pharmacist will work collaboratively with the health care team and patients to optimize medication therapy and quality, and economic outcomes. The Pharmacist communicates drug information, provides comprehensive medication management, optimizes preventative care, supports seamless transitions of care through reconciling of medications, and provides education to patients and providers.

Must be a graduate of an accredited College or School of Pharmacy with advanced training (Pharm.D).
Minimum of one (1) year of clinical practice experience and experience working collaboratively with other health care professionals required.
Experience should also include work in primary care including assessing patient medication regimen, educating patients, documenting patient notes and care plans in an electronic health record, and communicating effectively with an interdisciplinary health care team.
Completion of a PGY1 ASHP-accredited residency required (ambulatory care or equivalent practice experience preferred); PGY2 ASHP-accredited residency preferred.
Board Certified Pharmacotherapy Specialist (BCPS) or Board-certified Ambulatory Care Pharmacist (BCACP) preferred.
