Come and join us in the ChristianaCare Pharmacy - Dedicated to getting the right drug to the right patient at the right time!As the Clinical Pharmacist, Care Transition, you will primarily function as transition of care pharmacist in the inpatient clinical pharmacy. You will assist with the distributive functions of the pharmacy and coordinate clinical pharmacy functions for patients transitioning care from inpatient to outpatient care. In collaboration with providers and other allied health professionals, evaluate drug therapy based on acuity/potential for problems, and provide medication-related care for the purpose of achieving outcomes that result in quantifiable impact.PRINCIPAL DUTIES AND RESPONSIBILITIES:
