Job Description: Inpatient surgery pharmacist position with combination of decentral, central, and an opportunity for OR satellite care at Mayo Clinic Hospital in Rochester, MN. In this role, the pharmacist provides exceptional care to patients throughout their peri-operative journey; including pre-operative, intra-operative, and post-operative phases of medication management. Decentral clinical services consist of collaborative practice with multidisciplinary teams, identification of drug-related problems, provision and documentation of recommendations through progress notes regarding optimization of drug therapy, admission and discharge medication reconciliation, transition of care planning, participation on discharge rounds, intermittent surgical team rounds, and direct patient care. Responsibilities include drug therapy and disease monitoring, medication order review and verification, anticoagulation management, parenteral nutrition management, pharmacokinetic monitoring, antimicrobial management, and utilization of collaborative practice agreements. Distributive services occur from central pharmacy and OR satellites on a rotating basis to serve our 125+ OR suites, 73 of which are located at Saint Mary’s Hospital campus, for top anesthesiologists and world class surgeons as they provide innovative care across all three shields of Practice, Education and Research. Pharmacists are actively involved in the development of guidelines, protocols, and order sets. Positions will work a rotation between day and evening assignments. Previous experience required includes PGY1 or PGY2 training and/or equivalent experience of 3-5 years in practice.
