Serve as a resource to interdepartmental pharmacy staff liaison to pharmacy antimicrobial stewardship team. Assist with of antimicrobial therapy management
Provides comprehensive clinical pharmacy services to a specialty patient population, including participation in interdisciplinary team rounds, selection and dosing of medication therapies, therapeutic drug monitoring and adjustment of drug therapies.
Reviews, interprets and evaluates medication orders to optimize pharmacotherapy during order verification or entry in the electronic health record.
Performs and documents clinical activities/interventions and their outcomes for use in drug therapy quality improvement efforts.
Reviews literatures, develops and deliver evidence-based drug information and education to multidisciplinary team and patients.
Serves as subject-matter expert for specialty patient population. Contributes expertise to multidisciplinary committees/teams in specialty area. Activities include but are not limited to developing protocols/guidelines, medication use evaluations (MUEs), formulary reviews, and order sets.
Improves organizational performance by identifying and implementing clinical and/or safety initiatives
Serves as a preceptor and resource to the pharmacists, pharmacy interns and students. Provides continuing education programs and assist in the development and implementation of educational opportunities.
Actively participates in the development, implementation and ongoing coordination of pharmacy residency research projects and/or mentors the resident in the preparation of posters, local, regional or national presentations, or publications.
Clinical Specialist - Solid Organ Transplant
Maintain documentation related to transplant pharmacy activities that are compliant with CMS regulations
Attend and actively participate in inpatient rounding/planning/review with multidisciplinary team renal/heart transplant
Ensure continuity of care and minimize drug related problems
Attend and participate in all selection and living donor meetings
Assumes responsibility for the medication management of assigned patients post-transplant in both the inpatient and outpatient clinic setting
Monitor and evaluate medication therapy for appropriateness based on the patient's health status, institutional criteria /guidelines, and cost-efficient measures including medication reconciliation, medication monitoring and patient education
Remains integrally involved in the education of clinical staff, pharmacy residents, and pharmacy students
Provide guidance and coordination for order sets, formulary management, and other pharmacy related documents as needed
Evaluate potential transplant recipients for pharmacologic and non-pharmacologic risks
Provide pretransplant medication education to patients that are medically clear for transplant
Coordinate discharge medication reconciliation and medication delivery prior to patient discharge during their transplant encounter
Provide counseling to new transplant recipients prior to discharge, as well as provide patient with a comprehensive medication list and aid in filling the initial pill box
Coordinate development and implementation of drug therapy protocols, assists in ensuring protocol adherence and measure outcomes with these protocols
Provide education and training to members of the transplant team and practitioners in training as well as pharmacy staff
Facilitate cost containment strategies and pharmacotherapy optimization to maximize patient and center specific outcomes
Lead and assist with clinical and pharmacoeconomic research
Provide 24 h a day, 7 day a week pharmacotherapeutic support evidenced by daily documentation of activities in the patient’s medical record
Meet with patients post-transplant during clinic visits and continue with education on post-transplant care, ensure proper medication adherence, assess appropriateness of medication regimens, and facilitate accurate filling of a patients pill box
Assist in ensuring that medications are billed properly, and/or assist in enrolling patients in various assistance programs, distribution programs, and adhere to various REMs programs as needed
Provide anticoagulation and pharmacokinetic monitoring for transplant patients
