University Hospitals’ Postgraduate Year 1 (PGY1) Community-Based Pharmacy Residency is a full time, 12 month program that provides exposure to a variety of patient care experiences within community and ambulatory practice, as well as providing experiential and educational opportunities to enhance knowledge base and promote personal growth. Residents will have opportunities to develop their clinical skills through patient care services, such as medication therapy management, transitions of care, and immunizations, as well as obtain a teaching certificate and precept pharmacy students. UH Meds ambulatory services emphasize interdisciplinary collaboration within the Primary Care Institute (PCI) to improve patient outcomes and provide comprehensive care.
UH Meds supports patients in reaching their health goals through comprehensive medication management and population health. Residents work in Advanced Primary Care (APC) practices with physicians, nurses, behavioral health managers, and other patient care support personnel to create comprehensive care plans and ensure patient access to medications. Residents also support transitions of care through the Meds to Beds program and discharge counseling at UH Cleveland Medical Center.
Residents with UH Meds PGY1 Community-Based Pharmacy Residency have the unique opportunity to work with patients throughout their entire healthcare journey, across multiple care settings. UH Meds Ambulatory and Retail pharmacy services continue to grow and innovate delivery of pharmacy services that support patients from the inpatient to the outpatient setting. Residents in this program will serve on the frontlines of patient care, working at the top levels of their degree and license. Residents play a pivotal role in the creation and implementation of expanded services that provide quality and compassionate patient care.
Graduate of accredited College of Pharmacy with a Doctor of Pharmacy degree. Eligible for licensure in the state of Ohio. Submit letter of intent, curriculum vitae, three letters of reference, and your official pharmacy school transcripts. Candidates must follow requirements and instructions via Pharmacy Online Residency Centralized Application Service (PhORCAS) and National Match Services (NMS). International students work VISAs are not accepted. Out of State students must obtain Ohio interns license prior to start date.
