The PGY-2 Ambulatory Care Pharmacy residency was established in 2010 and reaccredited by ASHP in 2021.  The program focuses on chronic disease state management and exposure to diverse populations (clinical, social, and economic).  Residents will participate in a number of required rotation experiences which span a variety of ambulatory care settings, including Internal Medicine and Family Medicine Primary care (month-long and longitudinal), Anticoagulation clinic and on-call service, Cardiology (General, Advanced Heart Failure), Transitions of Care, Ambulatory Clinical Services Management, Clinical Precepting, and Outpatient Pharmacy staffing in the Meds-to-Beds service.  Elective rotations may include further experience in any of the core, required practice areas or others including Am
