Responsible for medication therapy management using in depth knowledge of patient care, pharmacy practice, pharmacotherapy, and technology. Coordinates with other health care professionals to ensure optimal patient care through utilizing best practices. Provides education and mentorship to patients, pharmacists and other health care providers.
Be forward thinking and a change agent; work to develop, implement, and expand oncology clinical pharmacy services in the ambulatory setting (prior ambulatory experience preferred).
Primary work is within Allina Health Cancer Institute Clinc in a collaborative care team setting.
Dedicated administrative time to support site and system initiatives provided.
Optimize the outcomes of patients with oncologic disease states by providing evidence-based, patient centered drug therapies, and monitoring for drug-related problems and medication cost containment.
Coordinate current patient issues and therapy recommendations within an interdisciplinary cancer care clinic.
Support research activities of the respective specialty team as requested Keep abreast of changing protocols and clinical pharmacy standards as they pertain to oncology, hematology, and other applicable areas of specialty.
Ensure compliance with the Board of Pharmacy, Joint Commission, and other regulatory agencies
Provides input to System Formulary Committee regarding oncology practice.

For the Minneapolis location - Clinical emphasis in Breast and Solid Tumor preferred.
Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) required
PGY2 Oncology residency experience or similar required
Licensed Pharmacist - MN Board of Pharmacy required upon hire or eligibility to be licensed within 30 Days
Board certification in pharmacotherapy or specialty area upon hire preferred
