Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising and collaborating with medical staff on issues concerning drug therapy. Acts as liaison, and contributes to the development and delivery of continuing education programs among Pharmacy, Patient Care Services, Medical Staff and students. Participates in interdisciplinary patient care rounds and monitors orders for appropriate use, dosage, drug interactions, etc. and documents clinical interventions. Provides drug information to the health care team. Provides education to pharmacy, medical and nursing staffs. Coordinates Anticoagulation Sub-Committee meetings and all related activities.
