This position will support the Bone Marrow Transplant (BMT) and chimeric antigen receptor T-cell (CAR-T) Teams in optimizing the care of their patients. Participation in BMT and CAR-T hospital interdisciplinary rounds, verification of chemotherapy and other medication orders for hospitalized and outpatient infusion BMT/CAR-T patients, Medication Management Services, Transitions of Care Services, and patient education are anticipated routine activities. The BMT Pharmacist Specialist will serve as a liaison between the Pharmacy and BMT/CAR-T practices, collaborating to continuously improve medication use quality and safety, and will actively participate in both HemOnc and Pharmacy education programs and research.
