Shift: 12p-8:30 pm primarily but will rotate to other shifts as needed, weekends, and holidays

The role you’ll contribute:
Clinical Pharmacy Specialist to develop and establish pharmacy services in the Emergency Department. The candidate will work closely with the pharmacy management, ER physician and nursing staff, and ER medical residency program to provide comprehensive clinical pharmacy services for the ED and all associated areas (Pediatrics, Trauma, Urgent Care, etc.) including pharmacokinetic and therapeutic consultation.

CLINICAL ACTIVITIES:
Assumes responsibility for the delivery of pharmaceutical care to patients in the Emergency Department. Coordinates the use of human, material, and informational resources such that timely, efficient, cost effective and appropriate care is consistently rendered.
Demonstrates proficiency in order processing. Reviews physicians' medication orders and clarifies or corrects orders with the physician. Reviews patient medication profiles to guard against drug sensitivities, contraindications, duplications, and major drug interactions.
Reviews all medication orders for prescribing, formulary, and therapeutic issues. Suggests appropriate formulary alternative(s) based on the patient's clinical parameters.
Consults and advises members of the emergency department staff on matters relating to specific drug therapy problems and monitors and acts on patient laboratory results. Provides in-depth drug information and other informational materials to members of the medical, nursing and other healthcare professionals. Provides patient discharge medication consultations based on department standards. Calculates drug dosages based on pharmacokinetic parameters.
Works collaboratively with other clinicians and health-care providers to implement and maintain innovative disease management programs and clinical pharmacy services.
Participates in the development of medication use management programs within the system including clinical guidelines, critical pathways, disease management and drug use programs.
Keeps informed of the action, adverse effects and proper usage of all new drugs as they are made commercially available as well as of all investigational drugs being studied by the institution.
Facilitates proper information transfer with regard to medication use for patients converted to inpatient status from the ED.
Focuses cost avoidance and cost savings due to medication use in the Emergency Department.
ADMINISTRATIVE ACTIVITIES:
Demonstrates ability in operational area with respect to time management, supervision of staff, problem identification and resolution, and decision-making.
Coordinates and supervises work-place activities and resources; promotes professional competence; assures that services rendered and work performed is in accordance with departmental standards. Advises, assists, and supervises the work of pharmacy technicians; assumes final responsibility for accuracy of their work.
Works with medical and/or nursing personnel to resolve problems involving pharmacy services.
Assists with the review and revision of departmental policies and procedures, as requested. Applies departmental policies and procedures to complex situations and identifies areas for which policies and procedures need to be developed, improved or revised
Qualifications
The expertise and experiences you’ll need to succeed:
Bachelor's degree in Pharmacy or Doctorate of Pharmacy required
Pharmacist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
PGY1 clinical pharmacy residency completion and significant direct patient care experience in the areas of critical care/ER/toxicology preferred
Completion PGY2 Emergency Medicine residency/fellowship training preferred.
Board certification and ACLS/PALS certification preferred
ICU/ER experience preferred
