St. Luke's Health System is seeking a Pharmacy Extern - Medication Historian to join our Transitions of Care/Pharmacy team.

Position Summary

We are looking for a dynamic Pharmacy Extern - Medication Historian to work alongside our team of highly skilled providers and pharmacists to assist with the accuracy and completeness of medication history collection and medication reconciliation. You will stay at the forefront of the latest pharmacy technology and will work in a fast-paced environment to provide the highest level of care to our patients. Join a department that is highly respected and critical to the Transitions of Care/Pharmacy team.

Shift times

Weekends, 10-hour shifts

Location

Primary location is the Meridian Medical Center. Must be able to work upon occasion in the Nampa and Boise Medical Centers

Responsibilities

• Actively demonstrate the organization's mission and core values, and conduct oneself at all times in a manner consistent with these values
• Perform independent work accurately with attention to detail
• Identify target patients admitted to hospital within last 24 hours and give priority to patients recently admitted
• Gather information about the patient's medications from family or directly from the patient in a manner that is professional, utilizing excellent patient interviewing skills
• Collect and compile a complete list of patient's home medications including over-the-counter drugs and herbal supplements including drug name, dosage, frequency, reason for use and last dose taken
• Consistently provide superior service by responding to physician and staff consults and/or patient and family requests
• Input all medication history information into the electronic health record
• Keep record of all work processed for intervention tracking and reporting
• Participate in staff meetings for the purpose of disseminating information and monitoring staff needs, concerns and suggestions
• Adapt to workflow changes alongside a growing health system
• Perform other duties and responsibilities as assigned

Qualifications

• Education: Enrollment in or recent graduation from an accredited college of pharmacy
• Experience: 0 years's experience; six (6) months' retail pharmacy experience preferred
• Licenses/Certifications: Valid pharmacy intern/extern license in the state of practice
