This pharmacist will join a team of critical care and transplant pharmacists providing exceptional care to patients in our CV surgery and thoracic transplant populations. Coverage spans CV surgery ICU as well as care of heart, lung, and multivisceral transplant patients. Clinical services consist of daily rounding on a multidisciplinary team, identification of drug-related problems, provision of recommendations regarding optimization of drug therapy, admission and discharge medication reconciliation, transition of care planning, and direct patient care on a rotating basis. Additional practice areas may include medical and surgical ICUs. Schedule consists of day/evening shifts and every 3rd weekend.
Job Responsibilities include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Precepting of the PGY2 Solid Organ Transplant and PGY2 Critical Care pharmacy residents and other pharmacy learners is expected. Additionally, pharmacists are active members in quality improvement, research activities, and education.
Involvement in scholarly activities including quality initiatives and multidisciplinary research is encouraged and supported through extensive institutional and departmental resources. Opportunity for academic appointment at hiring and progression through appointment ranks with years of service of employment. Graduate of an accredited College of Pharmacy. Doctor of Pharmacy (Pharm.D.) degree or equivalent experience. Equivalent experience includes at least two years of relevant pharmacy practice experience.
**Visa sponsorship is not available for this position; also, Mayo Clinic does not participate in the F-1 STEM OPT extension program** Previous experience including PGY2 residency or fellowship training in critical care, cardiology, or solid organ transplant, PGY1/non- transplant PGY2 residency plus 2 years’ experience in critical care, or thoracic transplant practice, or minimum of 5 years of critical care or thoracic transplant experience.

Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required. The following are preferred:
Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations in transplant
Relevant applicable pharmacy experience working in a progressive and collaborative practice environment
Experience and/or interest in teaching and/or precepting
Board Certification in Critical Care (BCCCP) or Solid Organ Transplant (BCTXP)
License to practice Pharmacy in the State of Minnesota or eligibility for licensure.
