Under the direction of the Clinical Pharmacy Manager provides pharmaceutical care in conjunction with other members of the healthcare team to ensure high quality, safe and cost effective therapy to ambulatory and Hospital-based patients. Functions as the Diabetes Education Coordinator and is responsible for assessing the learning needs of patients/families, planning and implementing the educational process with patients/families, evaluating the outcomes, and making appropriate referrals. The Diabetes Educator Coordinator also serves as a resource across the continuum in matters related to diabetes education.
Duties / Responsibilities:
Leads diabetes teaching, assessment, and monitoring of patients referred to the Diabetes Education Program.
Communicates patient progress towards goals and makes recommendations based on ADA Standards of Care to the referring physician1Maintains, documents and reports quality control and improvement data associated with the clinics including weekly and quarterly proficiencies on all Point of Care devices.
Maintains appropriate quality control data, Adverse Drug Effects (ADE) and weekly metrics.
Responsible for the diabetes education program and supervision of the program personnel in conjunction with the department manager.
Leads/participates in continuous improvement programs to meet Organizational, Regional and Pharmacy Department goals and strategies that pertain to anticoagulation or diabetes education/management and student/resident programs.
Provides/Participates in clinical case conferences, continuing education programs, and other staff development programs with pharmacy or other healthcare providers.
Completes initial diabetes assessment and devises an individual educational plan. Provides class instruction to patients in the diabetes education program and determines when patients do not meet classroom criteria. Provides individual instruction for diabetes education patients based on need.
Responsible for verifying input of all necessary statistics and completing and submitting all state and American Association of Diabetes Educators (AADE) reports on an annual basis.
Participates in the development of new patient care programs, clinics and/or research projects to enhance pharmaceutical care opportunities.8Updates any marketing materials and works to market ambulatory clinic services to potential new referring physicians. Updates educational materials as appropriate based on updated guidelines and protocols.
Makes appropriate decisions using good judgment to reduce drug costs and ensure efficacy and safety of drug therapy for the entrusted lives of SJMO.
Maintains all appropriate documentation necessary for program re-certification including updating all Diabetes Educator's credentials, continuing education, updating educational materials, and meeting all requirements.
Responsible for drafting, reviewing and updating policies and procedures, protocols and standard operating procedures for all ambulatory clinic practices.
Provides anticoagulation therapy management to all patients referred to the Pharmacy Managed Anticoagulation Service and from the Joint Care Center (JCC).
Assists with interviewing potential new pharmacist specialists, pharmacy students (Longitudinal Advanced Pharmacy Practice or LAPP) and pharmacy residents.
Assists with development of Anticoagulation and Diabetes Education annual budget preparation and assures appropriate registration and billing is completed in a timely fashion.
Minimum Education, Licensure / Certification and Experience Required.
A. Education
A Doctor of Pharmacy (PharmD) degree from an accredited College of Pharmacy is the minimum acceptable level of education.
B. Licensure / Certification
Registered Pharmacist possessing or is eligible for a Michigan Pharmacist, Controlled Substance and Approved Preceptor License.
Certified Pharmacotherapy Specialist or equivalent Ambulatory Specialist Certification within 1 year of hire.
Meets criteria of a diabetes educator or certified diabetes educator within 1 year of hire.
Maintains certification by the Department of Customer Industry Services upon certification.
Current Basic Life Support (BLS) certification.

Excellent customer service orientation skills necessary in order to deal effectively with various levels of hospital personnel, outside customers and community groups.
Interpersonal skills necessary to communicate effectively with a wide variety of hospital healthcare personnel and patients.

Able to deal with diverse patient populations.
D. Experience
Completion of an American Society of Health-System Pharmacists (ASHP) accredited PGY2 Ambulatory Care Specialty Pharmacy Practice Residency or a PGY1 Pharmacy Practice Residency with at least 2 years of Ambulatory clinical pharmacy experience.
Strong clinical background with experience in ambulatory care issues such as hypertension, diabetes, cholesterol management and anti-coagulation.
