Overview Atlantic Health System is seeking a per diem clinical critical care PD Pharmacist to manage and optimize the pharmacotherapy of patients in the intensive care units at Overlook Medical Center every 4th weekend.Responsibilities Essential Functions:Design, modify, and evaluate medication therapy regimens for critically ill patients based on a variety of disease states and patient-specific factors in the ICUParticipate in multidisciplinary rounds with the ICU team consisting of pulmonary critical care attending physicians, medical residents, nurses, respiratory therapists, and dieticiansPerform therapeutic drug monitoring (TDM) for vancomycin, aminoglycosides, anticoagulants, and anti-epileptic medications
