Clinical pharmacy specialist provides pharmaceutical care to veterans as a clinical pharmacy practitioner (CPP) with highest level of pharmacist clinical privileges and with minimal supervision. CPP will be a member of the Patient Aligned Care Team (PACT) at the Community Based Outpatient Clinic at Pocatello, Idaho. Although the primary duty station for the CPP is Pocatello, s/he may be assigned to serve as surrogate provider for CPP based at the Idaho Falls CBOC, on occasions Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
