This is a great opportunity for a highly motivated Clinical Pharmacist to work within a Primary Care team to support high quality patient care. This is a full-time position with variable hours between 7:00 am to 7:00 pm, Monday – Friday, highly flexible schedule.  No weekends or call. 

Primary Responsibilities:

• Embedded with primary care team to provide high quality care primarily to their delegated Medicare Advantage patient
• Provide medication management and direct patient care based on evidence-based guidelines, protocols, and disease management
• Performing patient medication reconciliation, medication therapy management (MTM), medication optimization and 5-star quality adherence work
• Work with providers and clinical teams to implement cost-effective medication interventions
• Provide clinical medication education and feedback for providers

Required Qualifications:

• PharmD, residency trained or 3+ years of clinical experience

California Residents Only: The hourly range for California residents is $85,000 to $167,300. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement.
