Midwestern University College of Pharmacy offers an ASHP-accredited PGY2 Ambulatory Care pharmacy residency program with a primary practice site at the Advocate Medical Group Southeast Center in Chicago, IL. The purpose of this program is to use and build upon the competencies achieved in a PGY1 residency to promote the development of proficient, independent clinicians with expertise in managing patients' pharmacotherapy in ambulatory care settings. Program graduates will gain experience providing patient-centered care through a comprehensive collaborative practice agreement for patients with chronic diseases, including but not limited to, diabetes, heart failure, hypertension, hyperlipidemia, asthma, and COPD. Residents will be integrated into a family medicine and internal medicine clinic and will work closely with providers with the opportunity to provide interdisciplinary and independent patient care visits. In addition, program graduates will have the skills necessary to initiate new ambulatory care services in their future career. This program also emphasizes developing teaching skills and prepares graduates to become effective educators in the clinical and academic setting. Residents typically spend 4 days/week at clinical sites and 1 day per/week on campus involved in college-related activities. Applicants must have graduated with the Doctor of Pharmacy degree, completed a PGY1 residency program and be eligible for pharmacist licensure in Illinois. The applicant must be motivated, self-directed and have excellent communication skills. An interest in teaching is essential.
