This ASHP-Accredited PGY-2 residency program in Solid Organ Transplantation is designed to develop a well-rounded clinical pharmacist with specialized clinical skills and knowledge of pharmacotherapy in many areas within adult solid organ transplantation. The resident will practice as an integral member of the transplant team providing pharmaceutical care for transplant patients in all phases of transplant, including evaluation, pre-, peri-and post-transplant. The resident is embedded in both the hospital inpatient units and well-established comprehensive ambulatory transplant clinics. Inpatient responsibilities include participating as a member of the primary patient care team from the time of transplant evaluation, active transplantation and through any subsequent hospitalizations for transplant recipients regardless of admission diagnosis.  Ambulatory responsibilities include conducting formal, billable MTM/CMR visits with pre- and post-transplant patients and living donors, performing therapeutic drug monitoring and serving as a drug information and overall pharmacotherapy resource to the multi-disciplinary team, patients and their caregivers.  The resident will give didactic lecture(s) and facilitate case studies in the transplant pharmacotherapy course at the University of Minnesota College of Pharmacy, and co-precept rotations for PGY-1 residents and pharmacy students on Advanced Practice Experiences. Research opportunities are exemplary, with longitudinal experiences available with the Scientific Registry of Transplant Recipients (SRTR) and United States Renal Data Systems (USRDS)which are housed on site.  The resident will be expected to submit a clinical research project to a national transplant conference. 

ASHP-Accredited PGY1 residency required, along with Minnesota licensure.
