Carilion Clinic is a six-hospital health system located in Southwest Virginia. The Clinical Pharmacy Specialist in Infectious Diseases will integrate into a long standing and progressive system-wide Antimicrobial Stewardship Program (ASP), including a robust ID Pharmacist team, ID PGY2 pharmacy residents and a collaborative ID Medical Division. The position is primarily based out of Carilion Roanoke Memorial Hospital (CRMH), which is a 763-bed academic medical center affiliated with the Virginia Tech Carilion School of Medicine. As an academic facility with a Level 1 trauma center, this tertiary care institution has numerous post-graduate training programs, including multiple medical and surgical residencies and fellowships. The ASP also supports a network of Carilion Clinic community hospitals, including Carilion New River Valley Medical Center (CNRV) and four non-24-hour smaller community hospitals. Both CRMH and CNRV have pharmacy residency programs.
This specialist will primarily focus on inpatient ID responsibilities, joining a team of colleagues that rotate coverage of CRMH stewardship, community hospital stewardship, and ID consult team services. The position involves direct patient care responsibilities including: prospective audit/feedback utilizing decision support software, penicillin allergy de-labeling/skin testing, and rapid diagnostic blood culture review. Administrative responsibilities will include leading system initiatives, infectious diseases guideline development, staff education, relevant committee representation, and precepting pharmacy students as well as PGY1 pharmacy residents (CRMH and CNRV locations) and PGY2 infectious diseases pharmacy residents (CRMH location). ?Collaboration is expected with the ID division at weekly case conference, as well as weekly team meetings with the system ASP team. CDC Core Priority Measures, NHSN antimicrobial usage data tracking, and key performance metric tracking are completed at the system level and would be supported by this role.
Qualified candidates must be a graduate of a College of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE) and must possess a current Virginia pharmacist license or be eligible for licensure. An ASHP-accredited PGY1 Pharmacy Residency is required and specialized PGY2 residency training in Infectious Diseases is preferred. Applicants must possess excellent leadership, communication, interpersonal and teamwork skills, as well as demonstrate advanced skills in critical thinking and analysis. Must be able to effectively budget time, work efficiently and accurately, and follow procedures with minimum supervision.

Carilion Roanoke Memorial Hospital is a Level I trauma center, tertiary/quaternary care facility, and Certified Primary Stroke Center with ~95,000 patients visits annually and over 2,000 trauma activations. It is currently licensed for 72 beds and has a dedicated Pediatric ED, Observation Unit, and Transitional Care Unit, along with 2 trauma bays. There is planned expansion to 120 beds and will have a dedicated 12 bed critical care pod, 40 bed observation unit, 5 trauma bays, and other dedicated treatment areas. ?Carilion Clinic is also affiliated with the Virginia Tech Carilion School of Medicine and has numerous post-graduate training programs, including multiple medical and surgical residencies and fellowships.
The Emergency Medicine (EM) Clinical Specialist provides comprehensive pharmaceutical care to adult and pediatric patients in a variety of acuity levels to assure safe and effective drug therapy. The Emergency Medicine Clinical Specialist will develop and maintain a clinical practice by participating in bedside rounds and responding to trauma and medical emergencies.
This daytime EM specialist position will work a modified 7 on/7 off schedule, alternating with an EM specialist counterpart. ?Activities include collaborating with members of the ED to develop policies and protocols that provide effective and efficient use of medications, providing consultation for other health care providers, educating patients and other health care providers, and reviewing and processing medication orders. This position would also include relevant committee representation, staff education, and precepting pharmacy students. The Emergency Medicine Clinical Staff II pharmacist will develop and maintaina clinical practice by participating in bedside rounds and responding to trauma and medical emergencies. The night shift EM pharmacist will work a 7 on/7 off night shift schedule, alternating with an EM pharmacist counterpart. Activities include collaborating with members of the Emergency Department, providing consultation for health care providers, educating patients/nurses/providers, optimizing medication inventory in ADC machines, and reviewing and processing medication orders. This position would be directly involved withprecepting a night shift learning experience for PGY1 pharmacy residents and PGY2 pharmacy residents in critical care, as well as supporting longitudinal residency precepting commitments such as professional presentations, research projects, and medication use evaluations.

The CRMH inpatient psychiatry location is licensed for 43 adult beds and 23 child/adolescent beds. The Clinical Specialist in Psychiatric Pharmacy provides comprehensive pharmaceutical care to adults, adolescents, and children in the inpatient psychiatry units to assure safe and effective drug therapy. The Clinical Specialist in Psychiatric Pharmacy will develop and maintain a clinical practice by collaborating with attending providers and residents on the inpatient psychiatry teams to assist with medication management of complex psychiatric disease states and chronic medical conditions. Rounding experiences include academic team-based rounds and interdisciplinary rounds. Facilitating continuity of medication-related care from admission through discharge is a focus. Additional activities include collaborating with the Psychiatry Department to develop policies and protocols that provide effective and efficient use of medications, educating patients and health care providers, reviewing and processing medication orders, and providing consultation for psychiatry providers in the ED and ambulatory settings. This position would also include relevant committee representation, staff education, and precepting pharmacy students. This position would be directly involved in precepting an elective rotation for PGY1 and PGY2 pharmacy residents.
