This is an Ambulatory Care Clinic position including rural Health Clinics, position will be based out of Kaweah Health Exeter Clinic. ExeterCA
The outpatient pharmacy department provides a complement of ancillary services such as meds-to-beds and an employee pharmacy. A home infusion pharmacy provides care hospice and other patients with home infusions. Ambulatory care pharmacists practice in outpatient clinics including a broad array of primary and specialty care to a largely rural and medical underserved community.
Outpatient ambulatory care clinics include:
Rural Health Clinics (RHCs) – Primary and specialty care practice serving rural, underserved areas of Tulare County
Family Medicine GME clinic located in the Tulare RHC – Formal clinical training site for Kaweah Family Medicine Residency providing primary care
Kaweah Health Specialty Clinic – Referral based, high-risk clinic for treatment and management of type-2 diabetes, COPD, infectious disease, rheumatology, pain management and other disease states requiring resource-intensive multidisciplinary care
Clinical Pharmacist – Ambulatory Care
Clinical Pharmacists are assigned to ambulatory care clinic settings for the management of patients’ chronic disease state management. Pharmacists provide individualized patient consultations including assessment, management and monitoring of patients in addition to precepting pharmacy students/residents and completion of patient care projects for the pharmacy department and organization.
Pharmacists practice under an organizational policy granting broad prescriptive authority and are credentialed members of the Kaweah Health Medical Staff.
This position will practice in the Rural Health Clinics with a focus on primary care disease states such as diabetes, tobacco cessation, hypertension, cardiology, polypharmacy, and other disease-states as referred by primary care providers.
Serves as the clinical expert in the area of ambulatory care pharmacy services providing pharmacotherapy recommendations and direct patient care services.
Provides both consultative and direct patient care services with prescriptive authority for referred patients via collaborative practice agreements in the ambulatory care setting.
Orders and interprets pertinent labs and patient assessments for the purposes of monitoring and managing pharmacotherapy regimens of patients to assess efficacy, safety, and toxicity.
Provides in-person and telephonic Medication Therapy Management (MTM) services.
Ongoing participation in multidisciplinary patient care rounds.
Provides timely and effective communication with referring providers and allied health staff regarding patient care activities according to district policy.
Participates in the development and implementation of proactive population management programs for high-risk patient populations.
Documents clinical interventions on an ongoing basis.
Serves as preceptor for pharmacy residents and students.
Serves as faculty for the Family Medicine Residency GME.
Provides didactic lectures and facilitates patient case discussions related to pharmacotherapy to family medicine residents and faculty.
