Job Description: Solid organ transplant pharmacist to provide exceptional care to transplant recipients and donors in an inpatient, decentral position. Current coverage spans heart, lung, pancreas, living and deceased donor kidney and liver transplants, and living donors. Clinical services consist of daily rounding on a multidisciplinary team, identification of drug-related problems, provision of recommendations regarding optimization of drug therapy, admission and discharge medication reconciliation, transition of care planning, routine use of collaborative practice agreements, and direct patient care on a rotating basis.
