Children’s Hospital of Philadelphia clinical pharmacy team is seeking motivated candidates for clinical pharmacy specialist and decentralized clinical pharmacist position(s).
Reporting to the Clinical Manager, the Clinical Pharmacy Specialist / Decentralized Pharmacist will provide clinical, administrative, organizational, distributive, and educational services in a manner consistent with the philosophy, mission and goals of the Department of Pharmacy and the Hospital. The ideal candidate will possess the clinical and technical knowledge and experience necessary to provide services to pediatric patients and their families in this dynamic and patient-centered environment. The individual must have the ability to function independently, educate/supervise students and residents, and develop and facilitate teamwork and confidence in the department.
Responsibilities of the position include: Develop and maintain a consistent clinical pharmacy practice that provides optimal pharmaceutical support to patients at CHOP in an appropriate, efficacious, safe, and cost-effective fashion; provide drug information and/or educational programs to healthcare professionals and patients; contribute to departmental and professional publications and programs; act as a liaison with nursing and medical staff concerning medication orders, drug and pharmaceutical details, patient reactions, adverse drug events and quality improvement concerns; provide timely communication to peers, supervisors and other healthcare professionals to keep them advised of events and problems; participate in hospital and departmental Continuous Quality Improvement activities (CQI), and utilize these principals to analyze and resolve problems in a systematic manner; enhance personal growth and development through participation in educational programs, in-services, meetings and workshops; precept pharmacy residents and students; perform duties that contribute to department of pharmacy operational goals and responsibilities; and contribute to training of new pharmacists and technicians.
