The University of Virginia Health is recruiting for full time pharmacists positions in pediatrics. This position supports pediatric pharmacy services for the 120 bed UVA Children's Hospital. The schedule for the position is two weeks of evening coverage supporting UVA Children's and two weeks of dayshift float clinical coverage. Coverage during day shift may include: PICU, NICU, Gen Peds, Heme/Onc (inpatient and/or ambulatory), Battle Infusion (sterile compounding), and operational shifts. This pharmacist will participate in the team weekend rotation (every 4th weekend) and intermittent nightshift backfill (4-8 nights/year after 1 year of service).

Job Posting – Clinical Pharmacist – Solid Organ Transplant
The Transplant Clinical pharmacist provides specialized patient-centered clinical services to the inpatient solid organ transplant population in addition to the outpatient transplant clinic. This pharmacist will participate in the team weekend rotation (up to every 4th weekend), intermittent surgery evening coverage (as needed) and intermittent nightshift backfill (8-15 nights/year after 1 year of service).

The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting as part of a multidisciplinary clinic at the UVA Cancer Center. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record.

The inpatient oncology pharmacy team at UVA is seeking an individual interested in working in the areas of inpatient malignant hematology, bone marrow transplant, and cellular therapy. The delivery of care by the inpatient pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams. Inpatient pharmacist provide ownership of preparing, releasing and coordinating chemotherapy and other anti-cancer orders for inpatient administration including CAR-T Therapies, other novel, high risk therapies, and supportive care plans. Pharmacist provider patient/caregiver medication education including: weekday meds-to-beds service with hematology/oncology pharmacist discharge counseling and new start chemotherapy and other anti-cancer therapy counseling. Pharmacists participate in provider and nursing education on oncology-specific pharmacology and therapeutics, and can mentor and precept PGY2 oncology pharmacy residents, PGY1 pharmacy residents, and pharmacy students. This pharmacist will participate in the team weekend rotation (up to every 4th weekend), oncology pharmacist on-call, and intermittent nightshift backfill (8-15 nights/year after 1 year of service).

The... UVA department of pharmacy has team members who practice in various areas ranging from administration and business services, clinical inpatient care, and ambulatory settings. The inpatient hospital pharmacy provides decentralized dispensing and clinical services to an average daily census of over 600 patients. Sterile compounding for patients occurs in a newly renovated state-of-the-art IV clean room. The department operates numerous pharmacist-run outpatient clinics, several outpatient dispensing pharmacies, a specialty pharmacy, mail order, and home infusion services. The entire department has extensive technology and automation to provide innovative and safe care. Recent outpatient pharmacy, outpatient surgical services with associated pre and post-op clinical pharmacy care, and community hospital expansion has allowed UVA to provide pharmacy services to an increased number of patients throughout the state.
In addition, the department has greater than 90 pharmacy student and resident preceptors serving our 12 ASHP-accredited residency programs as well as introductory and advanced experiential education of pharmacy students from Virginia Schools of Pharmacy.
