Overview
St. Luke's Health System in Meridian is seeking an Ambulatory Care Pharmacist to join our Ambulatory Pharmacy Department team at St. Luke's Family Medicine, Meridian.
The St. Luke's Ambulatory Pharmacy Department includes pharmacists embedded in clinics across the health care system within the following specialties: internal medicine, family medicine, endocrinology, anticoagulation, heart failure, lipidology, cystic fibrosis, and children's specialties.
Our ambulatory care pharmacy practice is the provision of integrated, accessible health care services by pharmacists who are accountable for addressing medication needs, developing sustained partnerships with patients, and practicing in the context of an outpatient clinic. It involves being an integral part of an outpatient, multidisciplinary healthcare team to improve quality and patient outcomes by focusing on medication management.In this position you will:
- Provide direct patient care under a broad collaborative practice agreement. This allows qualified ambulatory care pharmacist to initiate, adjust, and discontinue medications, order and monitor laboratory studies, and perform limited physical assessments for various conditions (e.g. diabetes, hypertension, hyperlipidemia, chronic obstructive pulmonary disease, asthma, heart failure, anticoagulation, etc.)

- patient education and counseling
- prospective or retrospective chart reviews and shared visits with other healthcare providers (i.e. physicians, nurses, behavioral health)
- Perform other duties as assigned
Responsibilities
- Review appropriateness of prescribed medication therapy
- Provide medication information and consultation to patients, nurses and medical staff
- Supervise and authorize the work of technicians, externs, interns and support staff
- Perform cognitive clinical pharmacy services and document findings and actions in accordance with standards of the medical center
- May assist in the training of technicians, externs, interns, residents and other department staff
- Serve as a role model and mentor to technicians, students, residents and new practitioners
- Perform other duties and responsibilities as assigned
Qualifications
- Valid Idaho Pharmacist License
Preferred qualifications:
- Clinical management of primary care based chronic diseases
- Completion of PGY2 Ambulatory Care Residency OR completion of a PGY1 pharmacy residency program with ambulatory-focus OR equivalent ambulatory care experience
- Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)
- Prior experience in establishing a new clinical practice
- Previous direct patient care experience under collaborative practice for clinical management of diabetes
