The role you’ll contribute:
Prepares and organizes pharmacy medications by name and dosage, as listed on patient orders
The value you’ll bring to the team:
·Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems
·Proficiently verifies/enters medication orders and proactively communicates with medical staff to resolve problem medication orders
·Provides accurate, safe and timely medication therapy
·Initiates and facilitates appropriate therapy based on age, disease, and drug considerations
·Promotes cost-effective drug therapy and actively participates in stewardship activities and restricted medication/surveillance programs Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist consults
Qualifications
The expertise and experiences you’ll need to succeed:
·One year of Contemporary hospital pharmacy experience
·Bachelor of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree
·Applicable State Pharmacy License
