The role you’ll contribute:  

Prepares and organizes pharmacy medications by name and dosage, as listed on patient orders

The value you’ll bring to the team:

·         Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems

·         Proficiently verifies/enters medication orders and proactively communicates with medical staff to resolve problem medication orders  

·         Provides accurate, safe and timely medication therapy

·         Initiates and facilitates appropriate therapy based on age, disease, and drug considerations

·         Promotes cost-effective drug therapy and actively participates in stewardship activities and restricted medication/surveillance programs Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist consults

The expertise and experiences you’ll need to succeed:

·         One year of Contemporary hospital pharmacy experience

·         Bachelor of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree

·         Applicable State Pharmacy License
