The PGY2 Geriatric Pharmacotherapy Residency at ACPHS is a one-year post-graduate program designed to develop leadership skills and provide training and experience in various areas of geriatric practice, research, teaching, and service. The resident will care for older adults across multiple practice settings, including home care, assisted living, long-term care, post-acute rehabilitation, PACE, and geriatric psychiatry. A unique aspect of the program is that the resident will work closely with geriatric medicine and nurse practitioner fellows from St. Peter’s Health Partners Continuing Care and Albany Medical College. This team of interprofessional learners serves as a model of best practice in caring for older adults. Residents will be integral to the geriatric pharmacotherapy consultation service and work on medication safety projects.
The ACPHS Teaching and Learning Certificate Program is optional for those who wish to enhance their teaching skills—residents who have not completed a teaching certificate. There are opportunities to participate in public health outreach, transitions in care, and local and national committees.
Be self-directed, motivated, and display a commitment to professional involvement
Have strong interpersonal and communication skills and a desire to provide patient-centered care to older adults in various practice settings.
Possess a Pharm.D. degree from an ACPE accredited School/College of Pharmacy.
Be eligible for a license to pharmacy in New York State.
Be a U.S. citizen or carry a permanent resident VISA.
Have completed, or expect to complete, a PGY1 ASHP-accredited residency program.
The resident must also complete their licensure within two (2) months of beginning the residency
