Principal duties of the Home-Based Primary Care (HBPC) Clinical Pharmacy Specialist are providing highly innovative and progressive clinical pharmacy services and developing and directing clinical pharmacy programs in the Home-Based Primary Care Program. Clinical pharmacy duties are also performed in other areas of the Medical Center as necessity requires and time permits. plans, coordinates, and conducts education and training programs for personnel. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Pharmacists appointed to direct patient care positions must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. ? 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. Grade Determination: Experience. In addition to the GS-12 requirements, which is Completion of an ACPE-accredited Pharm.D. program or 1 year of experience equivalent to the next lower grade level, you must have 1 year of experience at or equivalent to the next lower grade level (GS-12). Equivalent experience includes: Handling routine medication-related activities including, but not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; and providing oversight of technical staff in all aspects of medication distribution.. In addition to the GS-12 requirements, you must demonstrate the following knowledge, skills, and abilities: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. ["Major duties include, but are not limited to: Clinical Responsibilities Responsible for developing, organizing, coordinating, and managing all aspects of clinical pharmacy functions for HBPC, attends and participates in HBPC multidisciplinary rounds, team meetings, conferences, and other meetings as required for the purpose of making recommendations and providing consultation in all Pharmacy-related matters. Responsible for all HBPC patients registered in the program, must work independently and effectively to assure appropriate, accurate drug reviews are completed; interpret prescriptions and/or physician orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; reviews patient medication profiles, contacts physicians and makes authorized changes in orders or prescriptions as required. Functions independently in day-to-day clinical and administrative duties associate with the position, consulting with higher level supervisors only when required. In general, uses a high degree of appropriate professional judgment to solve complex problems, which routinely arise throughout the workday. Demonstrates recognized expertise in the area of geriatrics and the HBPC Pharmacy program. The HBPC Pharmacy Program Manager interacts with physicians and other health care professionals as demonstrated by regular attendance at HBPC multidisciplinary team meetings, medication review rounds, and other meetings for the purpose of providing consultation in all Pharmacy related matters. Demonstrates advanced knowledge of pharmaceutics, pharmacodynamics and pharmacotherapeutics as determined by clinical pharmacy specialist staff review. Documentation via CPRS Progress Notes and Consults of all medication reviews and interactions is required. Specific amount and time periods of documentation will be determined by JCAHO, HBPC, and /or other regulatory requirements. Demonstrates skill in monitoring and assessing drug therapy outcomes, including physical assessment and interpretation of laboratory and other diagnostic parameters. Educational Responsibilities Incumbent serves as a preceptor for pharmacy students on clinical rotations sponsored by the University of Iowa College of Pharmacy. Incumbent is a preceptor for VA Pharmacy Residents in the specialty residency program in Primary Care Pharmacy Practice rotations each year. Incumbent provides drug information and drug literature analysis as requested by the professional staff. Incumbent provides literature evaluations for the Pharmacy and Therapeutics Committee. Incumbent serves as consultant and active participant in drug utilization reviews and conducts and coordinates these reviews.; Quality Improvement, Administrative, and Research Writes medication use evaluation protocols, executes the study, and presents summarized data with appropriate recommendations to the Pharmacy and Therapeutics Committee. Develops, presents to P&T Committee for approval, and implements cost avoidance initiatives. Documents and reports to pharmacy management cost avoidance data annually, when assigned. Incumbent actively participates in evaluation of investigational research projects by serving as a subject matter expert for the P&T Committee's Research Subcommittee, when assigned. Incumbent shares responsibility for precepting/overseeing pharmacy residents' research projects, when assigned. In addition, incumbent is encouraged to pursue their own scholarly activities. Participates in team activities. Consistently collaborates with team and peers. Can be relied upon to carry out own responsibilities and assignments in a timely fashion.
