University Health’s infusion center is a multidisciplinary-led outpatient clinic that provides specialty infusion medications, including chemotherapy, biologics, immunologic, and supportive medications. Our clinical hematology/oncology pharmacy team’s responsibilities include, but not limited to:
Multi-disciplinary patient care visits (initial chemotherapy counseling, toxicity checks, etc.)
Therapeutic drug monitoring
P&T committee involvement
Creation of evidence-based institutional guidelines & quality initiatives/policies
Involvement in multi-disciplinary tumor boards
Role model/preceptor to pharmacy students, residents, technicians
Beacon validation & treatment plan maintenance
Expectations of position:
Provides specialized pharmaceutical care through collaboration with providers and nurses in the OPIC or MD/APP clinics to ensure the appropriate selection, dosing, administration and dispensing of medication orders
Review and assess medication orders to identify, prevent and resolve medication issues and provide therapeutic drug monitoring
Active engagement with Hematology/Oncology Pharmacy and Therapeutics (P&T) Subcommittee to optimize formulary, oversee judicious use of non-formulary medications, create clinical references, and develop processes and procedures to improve care of patients
Monitor, manage, and report adverse effects related to chemotherapy and biologic therapy
Demonstrate expert knowledge in specialty medications to apply to evidence-based clinical recommendations, REMs management, and sterile compounding and preparation.
Advance pharmacy services through research, and professional organizational involvement
Engage in professional self-development
Participate in pharmacy intern and residency training programs
Support the educational and professional development of the pharmacy department
