Clinical pharmacy specialists at University Health provide services to a wide range of adult acute care practice areas. Current openings include:
Critical care operational support – day/evening, 7 on 7 off
Emergency medicine – overnight, 7 on 7 off
Internal medicine – days
Solid organ transplant – rotating organs – days
Stewardship (Anticoagulation) – days
Stewardship (Endocrine/Diabetes) – days
Clinical pharmacy specialists at University Health are responsible for the provision of comprehensive clinical pharmacy services related to a specific area of practice to optimize safe, efficacious, and cost-effective drug therapy in accordance with all regulatory agencies. They serve as a pharmacotherapy expert member of an inter-disciplinary patient care team to provide direct patient care. Other general expectations include, but are not limited to:
Deliver all aspects of programmatic clinical services for patients in your practice area with emphasis on best practices, safety and quality initiatives, therapeutic drug monitoring, and disease state management
Provide comprehensive clinical pharmacy services for patients in your practice area to optimize safe, effective and cost-effective drug therapy including:
Multi-disciplinary patient care rounds
Patient, provider and nursing education
Protocol creation, and review
Precept pharmacist interns and/or pharmacy residents on clinical learning experiences
Provide support for and participate in Pharmacy & Therapeutic Sub-Committee(s)
Develop and maintain medication-related best practice guidelines and policies related to their specific area of practice
