PGY2 Ambulatory Care
This program will be physically present at the ASHP Midyear Conference and PPS. The PGY2 Ambulatory Care Pharmacy Residency is designed to provide a strong clinical foundation and help the resident further develop research, teaching, leadership, and writing skills. Clinical rotations are longitudinal in nature and include core and elective experiences that focus on direct patient care in the Patient-Centered Medical Home (PCMH) and specialty clinics. Each resident is offered the opportunity to customize their elective rotations to their clinical interest(s), selecting from opportunities in chronic kidney disease, cystic fibrosis, hematology/oncology, oral chemotherapy, pain/palliative care, solid organ transplant, specialty pharmacy, and transitions of care.
Required rotations: Independent Patient-Centered Medical Home (PCMH), Primary Care – PCMH Clinic (Internal Medicine and Family Medicine options; 2 longitudinal rotations), Anticoagulation, Clinical Administration, Medication Optimization, Psychiatry, Transitions of Care Clinic, and Research (always December)
Elective optional rotations (half day per month for 3-4 months): Chronic Kidney Disease, Cystic Fibrosis, Hematology/Oncology, Oral chemotherapy, Palliative Care, Solid organ transplant, Specialty pharmacy, and Transitions of care

Advanced Practice Pharmacist

POSITION SUMMARY
Under the administrative guidance of the Medical Director this position functions as a credentialed and privileged non-physician medical provider in a clinical pharmacist collaborative practice agreement (CPA) providing direct medical services. The Advanced Practice Pharmacist will be a key member of the medical team and will perform patient assessments, refer patients to other providers, and operate as a collaborative drug therapy management pharmacist.
ESSENTIAL FUNCTIONS
The Nottawaseppi Huron Band of the Potawatomi (NHBP) reserves the right to change, amend, add, delete and otherwise assign any and all duties, responsibilities, and positions titles as it deems necessary to meet the needs of the government. The statements below are intended to describe the general nature and level of work being performed by individuals assigned to this position. They are not intended to be an exhaustive list of all duties, responsibilities, and skills required of personnel so classified.
1. Provides direct patient care as a non-physician medical provider under the privileges and credentialing by NHBP and operates under a clinical pharmacist CPA in the identified area of specialization.
2. Participates in evaluation and management of diseases and health conditions:
a. Collects and reviews patient medical and medication histories
b. Performs patient assessment and assesses vital signs
c. Orders and interprets laboratory values
d. Facilitates ordering of medical equipment
e. Places referral for patient follow-up with other providers and specialty clinics/departments, as appropriate
3. Initiates, adjusts, modifies, or discontinues drug therapy within the scope of the collaborative practice agreement and area of specialization.
4. Designs and implements medication, monitoring, and adherence plans.
5. Provides disease state and medication education.
6. Provides care coordination and transition of care services.
7. Documents patient encounters, consultations, and prescriber interactions in electronic medical record according to all local, Tribal, State, and Federal policies and regulations.
8. Works in collaboration with all available resources, including medical, behavioral health, community health, and other resources outside of the department to provide optimal mediation management for patients within defined practice area.
9. Evaluates the clinical practice for patient care and monitors process for continuous improvement.
10. Participates in quality initiatives, projects, committees, policy development, and formulary management strategies.
11. Participates in research, publication, and presentations at State and National organizations.
12. Oversees pharmacy program within their area of specialization ensuring competencies for pharmacists, up- to-date protocols and drug therapy guidelines, and oversees quality assurance in defined area of practice.
13. Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
14. Complies with accepted standards of care, confidentiality and privacy laws, and policies and procedures of the NHBP in performing duties.
15. Participates in clinical team conferences, quality assurance, planning, and other types of meetings or activities designed to review, improve, enhance or expand services.
16. Maintains professional and technical knowledge by attending formal continuing medical education programs, participating in performance improvement activities, and doing self-study activities.
17. Maintains licensure in the State of Michigan by attending approved continuing education activities.
18. Coordinates leave and continuing education requests with the Medical Director and the other providers well in advance.
19. Maintains appropriate and healthy working relationships with staff, patients, and community members.
20. Represents the NHBP clinics and staff in a positive manner, within the community.

PGY2 Ambulatory Care Requirements
Interested candidates for PGY2 Ambulatory Care Residency must submit the following materials via PhORCAS no later than January 2, 2025:

Advanced Practice Pharmacist
MINIMUM QUALIFICATIONS
An applicant’s education, training and experience must be sufficient to demonstrate that the applicant possesses the ability to successfully perform each of the essential functions. The requirements listed below are generally representative of the education, experience, and skills and/or ability required to enable one to successfully perform the essential functions associated with his position:
1. Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited program.
2. State Board Licensed in the State of Michigan
3. Completion of a PGY1 Pharmacy Practice Residency or a minimum of one (1) year of clinically integrated patient facing practice
4. Must maintain a valid driver's license with good driving record; must be able to travel.
5. Must be able to successfully complete and pass a comprehensive background investigation and drug screening in accordance with NHBP standards.
6. Must be able to maintain strict confidentiality and have knowledge of HIPAA requirements.
7. Must be able to adjust schedule as needed or required.
8. Must possess excellent communication skills, both verbal and written.
9. Must always present in a professional demeanor and appearance.
OTHER SKILLS AND ABILITIES
1. Thorough knowledge of pharmacy and medical professional standards in addition to complete competency and proficiency of professional and clinical skills.
2. Knowledge and understanding of relevant physiology, pathophysiology, and disease states pertinent to scope of practice defined in collaborative practice agreement.
3. Possess a broad professional knowledge in the field of clinical pharmacy and aspects of the total pharmacy operations.
4. Basic knowledge of medicinal, chemistry, pharmacokinetics, pharmacology, microbiology, pathology and toxicology to independently perform professional assignments.
5. Knowledge of proper drug and medication storage to ensure perseveration and security.
6. Knowledge of potential side effects of certain drugs or medications to properly advise and counsel patients.
7. Skill in the assessment of patients and their drug regimens, the development of nonpharmacological and pharmacological therapeutic plans.
8. Ability to understand, gain knowledge and appreciate the differences with the Native American culture and customs.
9. Willingness to develop a comprehensive understanding of Trauma Informed Care and apply to daily functions.
10. Understanding of, willingness to learn, and ability to support programs and services that are culturally specific, cultural in nature, and culturally focused.

Michigan Medicine and The University of Michigan Health (UMH) includes three hospitals, approximately 30 health centers and 120 outpatient clinics, the UM Medical School and its Faculty Group Practice, and the Michigan Health Corp. Located in Ann Arbor, Michigan, the Health System maintains the first university-owned hospital in the United States and the oldest College of Pharmacy in a state-supported institution.

The Department of Pharmacy Services strives to see patients and their families through a number of services and locations. We serve our patients both in the inpatient and outpatient setting. When patients are in the hospital or receiving care at a UMH facility, pharmacists are there working with physicians and nurses to ensure best care. Pharmacists serve as experts in drug therapy and dosing. Pharmacists perform functions like making sure chemotherapy orders are correct and properly dosed, helping patients and physicians with anticoagulation dosing, and dosing for drugs such as antibiotics an...d anticonvulsants. The pharmacy also provides outpatient locations for patients to fill prescriptions. Because of the specialized care that UMH provides, often other community-based pharmacies are not equipped to handle some of the complicated prescriptions that patients receive.
