The Department of Pharmacy’s mission is to provide quality patient care by optimizing medication use and ensuring medication safety. Integrated throughout the hospital, the pharmacy team is committed to evidence-based medicine and collaborating with other healthcare professionals to provide the highest quality pharmaceutical services. Pharmacists are involved in physician-led critical care and general medical rounds, cost savings and quality improvement projects, drug therapy surveillance, medication order entry, preparation and delivery, precepting residents and APPE students, and ambulatory clinics.
The Cardiovascular Intensive Care Unit (CVICU) at Holmes Regional Medical Center is a 14-bed unit that cares for cardiovascular surgery patients. The CVICU is divided into two teams (Cardiothoracic Surgery and Intensivist) with two attending physicians who are responsible for the patients on their team. The CVICU pharmacist is integral in multidisciplinary rounds with these two care teams. Additionally, the CVICU pharmacist will be involved in precepting PGY-1 and PGY-2 Critical Care residents while on their Critical Care or CVICU rotation, leading topic discussions, and providing guidance on medication use evaluations and research projects.
To be fully engaged in providing Uncompromised Safety, Superior Quality, Memorable Patient/Customer Experiences, and Financial Stewardship, the associate clinical coordinator acts on a facility level to assist the system clinical pharmacy manager in planning, developing and coordinating clinical pharmacy services and ensuring that the department of pharmacy provides optimal clinical services on a daily basis at their institution. The Clinical Pharmacy Specialist facilitates the development, implementation, and maintenance of systems that promote desirable patient outcomes through the use of safe, cost-effective drug therapies.
PRIMARY ACCOUNTABILITIES: Engagement:
Assists in the implementation of HF mission and execution of the overall strategic goals as described by senior leadership
Acts under the direction of the system clinical pharmacy manager and as such, keeps him/her informed on the progress and status of objectives and action plans
In conjunction with system clinical pharmacy manager, is responsible for planning, implementing and evaluating goals and objectives for clinical pharmacy
Maintains a positive, open and effective relationship with the medical staffs, hospital management teams and all levels of personnel through open communication utilizing effective written and verbal skills.
Work diligently to increasingly engage, provide tools of engagement, and raise issues of engagement with pharmacy
Disseminates best practice information obtained from outside resources and across health first
Quality/No Harm:
Prepares and presents drug reviews, Medication Use Evaluation (MUE) criteria, antibiotic sensitivity reports, adverse drug reaction reports, and pharmacy interventions to appropriate committees
Reviews the use of targeted medications for compliance with current pharmacoeconomic programs
Facilitates the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees.
Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions, and appropriateness of medication therapy.
Maintains current pharmacist licensure.
Participates in quality improvement activities both within pharmacy and interdepartmental where appropriate
Collects and analyzes data and maintains documentation of clinical activities
Evaluates methodologies, systems and programs of clinical pharmacy/medication therapy and implements necessary changes to achieve maximum efficiency and effectiveness that will result in improved patient care.
Customer Experience:
Provides coordination of clinical pharmacy activities in order to maintain an environment that is conducive to the delivery of quality patient care while continuously seeking improvement in assigned areas.
Directs development, implementation, and maintenance of clinical pharmacy services.
Provides accurate, adequate, and timely drug information to the professional staff.
Provides training and education to physicians, pharmacists, nurses, etc. regarding pharmaceutical care and pharmacoeconomics.
Assists the system clinical pharmacy manager in supervising clinical pharmacy personnel. Including recruiting, interviewing and disciplining when necessary
Assists in the development of job descriptions for clinical pharmacy personnel. Evaluates and counsels staff on their performance.
Coordinates clinical pharmacy participation in community educational programs (e.g., diabetes classes, health fairs).
Contributes to the advancement of the profession through presentations at local and state meetings.
Coordinates policies and procedures for clinical activities (e.g., therapeutic interchange).
Establishes productive, collaborative relationships with staff members and within the community.
Attends management meetings and reads pertinent email communications.
Sustains professional competence and promotes development through participation in inservices, workshops, seminars, and professional organizations.
Develops and implements clinical pharmacy services in collaboration with associated department services
Stewardship:
Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff.
Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost effective therapeutic alternatives to medical staff as needed.
Reads and evaluates the medical literature for studies supporting appropriate pharmaceutical care and pharmacoeconomic programs.
Reviews the performance and documentation of clinical activities of all pharmacists including documentation of the financial impact of clinical activities.
Prepares reports on pharmaceutical care and pharmacoeconomic activities including financial impact, as directed.
Completes all competence/skills assessment requirements (see attached competence assessment/skills list).
Maintains Patient and department/hospital Confidentiality
Qualifications Required:
Graduated from accredited college of pharmacy. Must possess a Pharm.D. degree
Preferred ASHP Accredited Residency or 2 Years of Clinical Pharmacy Experience
License to practice in the state of Florida by the Board of Pharmacy
Hours may be long and irregular; includes evening, weekends, and holidays as necessary.
