Description The primary job responsibilities include but are not limited to the following: Develop collaborative, engaged physician and patient relationships to increase referrals and participation in the clinical pharmacy program Take initiative to appropriately optimize and prescribe new medications within a consult agreement or collaborative practice agreement (CPA) Ensure adequate patient follow-up and monitoring to improve chronic disease management, quality metrics, medication safety and adherence Increase prescription volume to our internal UH retail pharmacies and/or home delivery Adapt to evolving prescription benefit management within our health pl
