The clinical ambulatory pharmacist will work as part of a multidisciplinary team and independently to provide care for patients who have chronic conditions. The pharmacist will be responsible for answering drug information questions, providing medication related education to patients, completing medication therapy management (MTM) opportunities and optimizing patient medication regimens. The pharmacist will be expected to have or be willing to obtain certification in the area of practice. This position may be responsible for providing pharmacy services remotely for off-site clinics.
This position is primarily 7:30 a.m. – 4 p.m. Monday through Friday.
PharmD and ASHP PGY1 residency and/or PGY2 specialty residency preferred.
The starting pay for this position was recently adjusted. The minimum salary for this position will now be $104,697/year. All pay rates will be based on candidate’s experience.
