Responsibilities

Provide safe, effective, evidence-based medication therapy management.

Collaborate with providers to optimize patient outcomes.

Educate patients and healthcare staff on medication use, dosing, and side effects.

Ensure compliance with all state and federal pharmacy laws and organizational policies.

Participate in quality improvement initiatives and patient care projects.
