The Solid Organ Transplant Clinical Pharmacy Specialist will serve as a leader within the pharmacy division and work closely with multiple pharmacists, providers, nurses, and other staff. This role serves as the pharmacy liaison to the three SOT teams, heart, kidney, and lung transplant in both the inpatient and outpatient spaces. Clinical Pharmacy Specialists also provide administrative support to the division and participate in various department and hospital wide committees. This includes creation and review of policies, procedures, and guidelines within their specialty area, oversight of formulary management, and assistance in training of clinical staff and leaders including competency assessments. This role will also provide operational support, clinical on call coverage, and weeke
