As a Clinical Pharmacist, you will be part of the Department of Pharmacy Hematology/Oncology team, as well as multidisciplinary teams within the University of Michigan Rogel Cancer Center. This pharmacist will:
- Provide care to ambulatory patients seen in the leukemia clinic, as an integral member of the multidisciplinary team. Collaborate with providers on crafting and personalization of treatment plans for patients with acute and chronic leukemias as well as other malignancies (e.g., myeloproliferative neoplasms, aplastic anemia, and other rare leukemias).
- Guide the incorporation of new treatments in a standardized evidence-based fashion, in conjunction with other members of the leukemia pharmacist team, including active involvement at weekly leukemia tumor board discussions.
- Coordinate care to promote outpatient delivery of treatments for acute leukemia.
- Collaborate closely with the inpatient leukemia team and BMT and cellular therapy teams to provide patient-centered care.
- Provide independent visits with patients to educate on complex treatment regimens and provide supportive care.
- Practice under a Collaborative Practice Agreement for nausea, vomiting, constipation, and diarrhea, as well as participate in a nausea and vomiting patient-reported outcome text messaging program.

You will ensure safe and evidence-based, cost-effective use of medications, as well as contribute to formulary review and maintenance.
You will assist in the development of staff educational and training programs within the Pharmacy Department
As a Clinical Pharmacist, you will also be appointed as an adjunct faculty member in the University of Michigan College of Pharmacy, where you will participate in the didactic and experimental training of pharmacy students and residents
You will work individually and/or collaboratively on scholarly/research activity
Work Schedule
This position is full-time, working Monday through Friday on day shift.

Requirements:
A Doctor of Pharmacy degree or an equivalent combination of education and experience.
Current license to practice pharmacy in the State of Michigan (including a Controlled Substance license), verifiable in LARA; or eligibility to become licensed within 90 days of employment start date.
A certificate of completion from an oncology residency, fellowship, or equivalent combination of education, training, and experience is necessary.
Experience in clinical pharmacy practice is necessary.
Board Certification in Oncology Pharmacy is desired, but not required

Michigan Medicine and The University of Michigan Health (UMH) includes three hospitals, approximately 30 health centers and 120 outpatient clinics, the UM Medical School and its Faculty Group Practice, and the Michigan Health Corp. The Department of Pharmacy Services strives to see patients and their families through a number of services and locations. We serve our patients both in the inpatient and outpatient setting. When patients are in the hospital or receiving care at a UMH facility, pharmacists are there working with physicians and nurses to ensure best care. Pharmacists serve as experts in drug therapy and dosing. Pharmacists perform functions like making sure chemotherapy orders are correct and properly dosed, helping patients and physicians with anticoagulation dosing, and dosing for drugs such as antibiotics an...d anticonvulsants. The pharmacy also provides outpatient locations for patients to fill prescriptions. Because of the specialized care that UMH provides, often other community-based pharmacies are not equipped to handle some of the complicated prescriptions that patients receive.
