Clinical Pharmacist Specialist | Ambulatory Oncology/Hematology | Hybrid Position | $10,000 Sign On Bonus
This position may work remotely up to two (2) days per week.
The Clinical Pharmacist Specialist provides services for Oncologic and Hematologic conditions in the ambulatory setting. The pharmacist in this position provides extensive, direct pharmaceutical care in collaboration with providers in the Cancer Service Line, participates in drug therapy consults, clinical research protocols, and provides patient/family/staff/physician education, and precepts pharmacy residents and students. This position is a key clinical role supporting our Specialty Pharmacy program. It is embedded within the practice working with the Providers.
Principal Duties and Responsibilities:
Review and interpret physician’s medication orders, detect and evaluate adverse drug effects, evaluate drug efficacy, therapeutic goals and indications, review for contraindications/precautions, the patient’s current status of health, and review drug profiles and patient’s medical record for drug-drug, drug-food, drug-laboratory and drug-disease state(s) potentials.
Provide individualized patient education/evaluation regarding drug therapy as requested or identified by patient need.
Work collaboratively with pharmacists and pharmacy technicians within the Specialty Pharmacy, with the specialty pharmacy access concierge to help facilitate a smooth prescription processing process, and with the call-center staff to help appropriately address patient calls.
Monitor drug therapy outcomes for targeted patients within established guidelines.
Manage patients’ supportive care plan with a focus on pain, neuropathy, nausea, or vomiting
Develop and revise evidence-based guidelines and order sets to ensure standardized, safe, and effective treatments regimens.
Participate in quality assurance/quality improvement activities as needed to achieve and maintain URAC and ACHC accreditation and adherence to legal and regulatory requirements
Represent Department of Pharmacy Services on committees as assigned
Education and Experience Requirements:
PharmD from an ACPE-accredited School of Pharmacy required
ASHP-accredited PGY1 Pharmacy Practice Residency required or equivalent experience (3 years of clinical pharmacy experience)
ASHP-accredited PGY2 Oncology Pharmacy Residency required or equivalent experience (3 years of clinical pharmacy experience, in addition to PGY1 residency or equivalent)
Experience in ambulatory oncology pharmacy practice preferred
Board Certified Oncology Pharmacist (BCOP) required within 2 years of hire
Current licensure (or eligible to reciprocate) in the State of Delaware
May require additional state licensure to provider virtual care to patients
Pharmacist | 340B Coordinator
In this role, you will ensure compliance with federal regulations, oversee program operations, and collaborate across departments to optimize cost savings and patient care.
Principal Duties and Responsibilities:
Serves as the institutional compliance expert on 340B regarding program details, policies, and procedures of the virtual inventory processes required for mixed-use and retail areas
Maintain policies and procedures to remain in compliance with HRSA
Responsible for the maintenance of the pharmacy split billing software
Monitor the purchases and replenishment of medications from the 340B, GPO, and pharmacy WAC accounts to ensure that there is no diversion of drug product
Maintain drug databases to reflect changes in drug formulary status
Review annual inventory of 340B purchased agents to ensure that we meet HRSA requirements
Review and refine 340B cost savings report in coordination with members of finance and internal audit, detailing purchasing, and replacement practices as well as dispensing patterns
Monitors ordering processes, integrating most current pricing from wholesaler (price updates occur quarterly), and analyzes invoices, shipping, and inventory processes
Develops and maintains internal relationships (accounting, legal, finance) and external relationships (wholesalers, manufacturers, vendors, pharmacies and third-party administrators) as needed for the 340B program
Continuously monitor max and par levels to effectively balance product availability and cost-efficient inventory control
Maintain awareness of deficiencies that have been observed at other facilities during recent HRSA audits and establish mitigating strategies to prevent failure points
Education and Experience Requirements:
Bachelor’s degree in Pharmacy required; PharmD preferred
Current Delaware pharmacist license (or eligibility to reciprocate)
At least 3 years of hospital pharmacy experience required
Supervisory experience preferred
Strongly preferred: Experience with 340B program administration
Willingness to travel occasionally for conferences, meetings, or site visits
Clinical Pharmacist | Elkton, MD | $10,000 Sign On Bonus
This position is full-time (40 hours weekly, 8-hour shifts) that primarily works day shift with a midnight shift component and a weekend/holiday requirement; currently every 4th weekend and 1-2 holiday shifts per year.
As a Clinical Pharmacist you will:
Participate in direct patient care through daily interdisciplinary rounds.
Manage specific medication therapies per approved policies (e.g., warfarin, aminoglycosides, vancomycin, IV to PO conversions, renal dose adjustments, indication-directed dosing adjustments, selection of surgical prophylaxis medications)
Evaluate drug efficacy, monitor therapeutic goals and outcomes, and review for contraindications/precautions
Order and evaluate laboratory tests per approved protocols to support medication therapy monitoring
Identify opportunities during chart review for counseling and medication history completion
Communicate recommendations directly with provider
Electronically document interventions and provide sign out as appropriate
Opportunities to serve as a preceptor for both pharmacy students and residents
Provide drug information to allied healthcare professionals and fulfill provider-initiated pharmacy consults
Assist with projects such as MUEs, drug monographs, and performance improvement activities
Review and interpret provider orders and dispense medication
Coordinate and supervise work-place activities
Education and Experience Requirements:
Bachelor of Science degree in Pharmacy or PharmD degree from an accredited College of Pharmacy required

This position is full-time (80 hours biweekly, 8-hour shifts) that primarily works day shift with an evening shift component and a weekend/holiday requirement; currently every 4th weekend and 1 - 2 holiday shifts per year.
