Assists in the definition and collection of monthly departmental volume statistics, as well as the distribution of monthly pharmacy performance metrics.           Assists with the maintenance of the pharmacy drug dictionaries, cost updates and pharmacy charge masters. Understands and is able to explain the internal functions of the system.           Creates accurate and useful reports through utilization all pharmacy decision support tools.           Analyzes financial, clinical and operational data to identify and quantify variances and opportunities as related to purchasing, formulary and billing of medications across the BHS system.           Prepares specified care line reporting and assists with the preparation of related analysis and commentary.           Supports assigned operational leaders in their understanding and completion of the annual operating budget.           Assists Pharmacy Director with all aspects of implementation, compliance, education, coordination and optimization of BHS 340B program.           Assists pharmacy leadership with a variety of projects, initiatives, and responsibilities as required, including Pharmacy Technicians and Pharmacy Buyer duties as needed. FLSA:               NON-EXEMPT                   DUTIES AND RESPONSIBILITIES:             Collaborate with hospital discharge planners and providers throughout the state about services provided at Benefis Teton Medical Center in order to provide appropriate referrals and transfers of patients to BTMC for post acute rehabilitation and recuperative needs.           Collaborate with providers, nursing staff, therapists, and DON for appropriate placement and transfer of patients to TMC swing bed or ELF status.           Collaborate with Business Services staff assisting with any financial concerns and coordination of financial assistance.           Screen hospital, swing bed, or ELF patients/residents within 24 hours of admit to identify complex patients who need discharge planning services and document in the EHR. With the exception of weekend admits, which will be screened on Mondays.           Comprehensively assess patients for functional abilities, illness and self care education needs, community resource needs, socioeconomic status and appropriate placement needs. Communicate and coordinate with nursing staff for assistance as necessary.           Work collaboratively with providers, therapies, nursing dietary, social work, business office, home care, and patient and families to develop discharge readiness goals and plans.
