Description of Medical Center: MemorialCare Long Beach Medical Center (LBMC) and Miller Children’s and Women's Hospital Long Beach (MCWHLB) have a combined 845-licensed beds and a combined average daily census of approximately 550 patients.  Long Beach Medical Center provides patient care services to emergency, critical, acute medical, surgical, and rehabilitation patients. Specialty areas include Memorial Heart and Vascular Institute, Todd Cancer Center, Level 2 Trauma Center, Wound Care Unit, and Rehabilitation Hospital.  MCWHLB provides specialized services to both pediatric patients and Center for Women patients.  Specialty services include a regional, tertiary care referral center for the treatment of high-risk pregnancies and newborns and the Jonathan Jaques Cancer Center. Inpatient pharmacy services are provided by a staff of ~60 pharmacists and ~75 support staff.  On the inpatient side, medications are provided using automated dispensing cabinets as well as a cassette system.  Cassettes are filled utilizing a robot and state of the art packaging equipment.  Intravenous solutions are made in a USP 797/800 certified clean room, and automated compounders are used for complex solutions.  In addition to the central pharmacy, medications are provided from 3 inpatient satellites (2 ICU satellite pharmacies). LBMC and MCWHLB have implemented an electronic medical records and CPOE featuring the Epic System since late 2009.  Pharmacy school teaching affiliations for Advanced Pharmacy Practice Experiences (APPEs) exist with the University of Southern California, University of California, San Francisco, Western University, University of California, San Diego, and University of the Pacific.  The pharmacy department has provided residency programs since 1972.  In addition to having one PGY2 administrative pharmacy resident, there are 4 PGY1 pharmacy residents. The medical staff consists of approximately 1,200 physicians covering all medical specialties.  Physician residents in training are represented in areas of critical care medicine, family practice, surgery, pediatrics, OB-GYN, and pathology.  An affiliation exists with the University of California, Irvine, School of Medicine.  Projects: There will be long and short-term projects that will encompass all areas of the inpatient pharmacy.  The resident is expected to present one major project at Western States Conference for Pharmacy Residents, Fellows, and Preceptors. Pharmacy Administration: The residents will meet with the Executive Pharmacy Director, Pharmacy Managers and other members of the pharmacy management staff to discuss contemporary issues related to pharmacy service.  Topics may include implementation/management of pharmacy services, Pharmacy and Therapeutics Committee-related information, budget preparation, drug procurement and inventory control, drug distribution systems, personnel management, and pharmacy practice issues.  During this year, the resident will become knowledgeable in the unit dose drug distribution systems, quality improvement programs, and pharmacy involvement with other hospital departments. Position Summary The Pharmacy Technician Per Diem works cooperatively with pharmacy staff to provide safe, accurate and timely delivery of medications to patients.  Under the direct supervision of a pharmacist, the Pharmacy Technician Resource processes new orders, accesses appropriate patient information in the electronic health record (EHR), issues drug doses to nurses, fills unit dose cassettes and automated dispensing cabinets (ADC), packages and compounds patient doses and maintains drug inventories. Essential Functions and Responsibilities of the Job Has the ability to triage medication requests and fill new medication orders accurately and efficiently, organizing his/her work to allow the pharmacist to efficiently check the medications prepared.  Understands the laws governing the maintenance of appropriate drug control for all drugs, including Schedules II-V.                                    Able to effectively use computer and automation resources relevant to their practice area to accurately provide medications in a timely manner and ensure accurate billing.                                Able to accurately fill patient medication cassettes, automated dispensing cabinets (ADCs) or drug delivery bins and/or restock ADC medications in a timely manner as relevant to their practice. Able to deliver/exchange cassettes/drug delivery bins and/or restock ADC medications in a timely manner as relevant to their practice area.                                 Able to order and restock inventory to maximize inventory turnover, prevent stock-outs, and minimize expired drugs.  Ensures correct storage conditions for medications, documents temperature logs, checks for expired drug products and other defects and removes them from inventory.                         Able to extemporaneously prepare unit dose packages, compound oral solutions and prepare uncomplicated IV admixtures or patient-specific unit dose injections using proper aseptic technique and following USP 797 standards as relevant to their practice area. Position Summary 6 Month temporary assignment with possible extension The Clinical Pharmacist Specialist is responsible for improving therapeutic outcomes for our patients through safe, rational and cost-effective pharmaceutical care, education and research.  The Clinical Pharmacist Specialist maintains competency in drug therapy and disease-state management in order to provide this care and improve patient care outcomes.  The Clinical Pharmacist Specialist provides drug information to physicians, nurses and other health care workers to improve the quality of medication prescribing and administration.  The Clinical Pharmacist Specialist assists with medication reconciliation, identifies potential medication safety issues and works actively to prevent medication errors. The Clinical Pharmacist Specialist educates patients and families to improve compliance with drug therapy as required.  The Clinical Pharmacist Specialist directs the work of ancillary personnel as necessary. The Clinical Pharmacist Specialist also contributes to the future of the profession and the pharmacy services within the hospital by teaching pharmacy students and post graduate pharmacy residents. Essential Functions and Responsibilities of the Job Medication Order Management:   The Clinical Pharmacist Specialist reviews medication and IV therapy orders and patient profiles; is able to identify the appropriateness of prescribed therapy relative to individual patient requirements and current departmental policies and procedures; and takes appropriate action to clarify or change inappropriate or incomplete medication and IV therapy orders. Communication: The Clinical Pharmacist Specialist communicates appropriately with clear and concise information to physicians, nurses and patients to improve the safety or effectiveness of a patient’s medication or IV therapy. Drug Information:   The Clinical Pharmacist Specialist demonstrates a solid working knowledge of common drugs, indications, dosages, monitoring parameters, pharmacokinetic principles, common medical conditions, clinical diagnoses, and the cost complication of common treatment regimens.  The Clinical Pharmacist Specialist demonstrates the ability to conduct a thorough patient medication history, to be able to enter it correctly into the electronic medical record,  and is able to accurately review a medication history obtained by a pharmacy technician. Drug Therapy Management:   The Clinical Pharmacist Specialist demonstrates competency in drug therapy guidelines; makes appropriate decisions and changes in pharmacy-regulated drug and IV therapy within current policies and guidelines; and/or makes appropriate recommendations to physicians or other prescribers when indicated. Financial Management:   The Clinical Pharmacist Specialist demonstrates proficiency in the area of pharmacy operations, patient care and patient safety activities which measurably improves patient care outcomes, reduces, overall cost of care, and/or improves the efficiency of delivering pharmacy services at Saddleback Memorial Medical Center. Supervision, Collaboration, and Teaching:   The Clinical Pharmacist Specialist directs the daily activities of pharmacy technicians and other ancillary personnel; works collaboratively with all health care team members in an appropriate and effective manner in order to optimize the productivity of the pharmacy; and participates in training/orientation of new staff, residents and students in service area.
